Myotropic antispasmodic. Inhibits FDE, causes accumulation of cAMP in the cell and reduces intracellular calcium content. Reduces the tone of smooth muscles of internal organs (gastrointestinal, respiratory, urinary, genital system) and blood vessels. Causes dilatation of arteries, increases blood flow, including cerebral. It has a hypotensive effect.
In high doses it reduces excitability of the heart muscle and slows intracardiac conduction.
In medium therapeutic doses, the effect on the CNS is weak.
Pharmacokinetics
Bioavailability is 54%. Binding to plasma proteins is 90%. Well distributed in the body, penetrates through histohematic barriers. It is metabolized in the liver.
T1/2 – 0.5-2 hours. It is possible to increase up to 24 h.
Extracted by the kidneys as metabolites. Completely eliminated from the blood by dialysis.

Indications
Hypertension (high blood pressure), Gallstone disease, Urinary stone disease, Biliary dyskinesia, Pain, Bronchospasm, GI spasms, Colitis, cholecystitis, ColicSpasms of smooth muscles of abdominal organs, bronchi, peripheral vessels, brain vessels, kidneys; angina pectoris (in combination therapy).
As an adjuvant for premedication.

How to take, the dosage
Over the body – 40-60 mg 3-5 times a day. Rectally – 20-40 mg 2-3 times a day.
In case of intravenous, p/u or intravenous administration, a single dose for adults is 10-20 mg; the interval between injections is at least 4 hours. For elderly patients the initial single dose is not more than 10 mg. For children aged 1-12 years the maximum single dose is 200-300 mcg/kg.

Interaction
Concomitant use with anticholinergic agents may increase the anticholinergic effects.
It is believed that there is a risk of priapism when used concomitantly with alprostadil for intracavernous administration.
There have been reports of decreased efficacy of levodopa when used concomitantly.
Limits the hypotensive effect of methyldopa.
Special Instructions
Use with caution in conditions after craniocerebral trauma, chronic renal failure, adrenal insufficiency, hypothyroidism, prostatic hyperplasia, supraventricular tachycardia, shock conditions.
V/v should be administered slowly and under a physician’s supervision.
Alcohol should be avoided during treatment.

Contraindications
AV-blockade, glaucoma, severe hepatic insufficiency, old age (risk of hyperthermia), children under 6 months of age, hypersensitivity to papaverine.

Side effects
Nausea, constipation, somnolence, increased sweating, arterial hypotension, increased liver transaminase activity.
In case of rapid IV administration and when used in high doses: development of AV-blockade, cardiac arrhythmias.
