Pharmacodynamics
The drug Ibuprofen refers to non-steroidal anti-inflammatory drugs for external use. Ibuprofen has anti-inflammatory and analgesic effects. It suppresses the production of inflammatory mediators. Inhibits cyclooxygenase and blocks the biosynthesis of prostaglandins.
Inflammatory activity is caused by the normalization of increased vascular permeability, improvement of microcirculation, decrease of histamine, bradykinin and other inflammatory mediators liberation, ATP formation inhibition, which causes decrease of energy supply of inflammatory process. Analgesic effect of ibuprofen is associated with a decrease in the intensity of inflammation, reduction of production and weakening of bradykinin algogenicity. Dimethyl sulfoxide has a local non-specific anti-inflammatory, analgesic effect, promotes better and deeper penetration of ibuprofen into the tissue.
The ibuprofen ointment has a fast and pronounced analgesic effect – it relieves the pain syndrome, including arthralgia at rest and on movement. Reduces swelling and hyperemia. Reduces morning stiffness and limitation of mobility in the joints.
Pharmacokinetics
In local application as 5% ointment ibuprofen is slightly absorbed and has no pronounced systemic effect on the body. It penetrates slowly into the joint cavity, but retains in synovial tissue, creating higher concentrations in it than in plasma. It undergoes biotransformation to form three main metabolites.
The products of metabolic breakdown are excreted from the body by the kidneys. Dimethylsulfoxide penetrates at a high rate through biological membranes, including intact skin, mucous membranes. It is metabolized to form dimethylsulfone and dimethylsulfide. Unchanged dimethylsulfoxide and dimethylsulfone are excreted in the urine and feces, dimethylsulfide is excreted through the lungs and skin.

How to take, the dosage
The drug in the form of an ointment is used topically.
A 5-10 cm strip of the ointment is applied to the skin in the area of pain and rubbed in gently. The ointment is used 3-4 times a day every day.
The duration of treatment is determined individually, depending on the severity of the disease and the nature of the injury.
Interaction
No cases of negative drug interactions have been found when prescribing ibuprofen ointment with other drugs included in the conventional treatment regimens.
Caffeine increases the analgesic effect of ibuprofen.
Special Instructions
The drug is intended for external use only.
In severe cases, it is recommended to combine topical therapy with oral administration of nonsteroidal anti-inflammatory drugs.
You should avoid getting ibuprofen ointment on the mucous membranes or in the eyes.
The occurrence of severe redness and itching indicates hypersensitivity to the drug (use is not recommended).
Contraindications
Individual hypersensitivity to the components of the drug, including dimethyl sulfoxide, ibuprofen and other non-steroidal anti-inflammatory drugs.
Moque dermatoses, eczema, infected abrasions and wounds, pregnancy, lactation.
Side effects
As a rule, there are no adverse effects when using the drug.
There may be skin hyperemia, a burning or tingling sensation.
In rare cases, allergic reactions (skin rash, itching, Quincke’s edema) may occur. If allergic reactions occur, treatment with the drug should be stopped; if necessary, desensitization therapy should be carried out.
Overdose
There have been no cases of overdose when using ibuprofen ointment.
In case of long-term use in high doses (higher than recommended) on extensive areas of the lesion, the development of systemic side effects of ibuprofen is possible.
Pregnancy use
It is contraindicated in the III trimester of pregnancy.
The use of the drug in I-II trimesters of pregnancy should be closely monitored by a physician.
