Indications
Acute respiratory diseases with symptoms of rhinitis (runny nose); Acute allergic rhinitis; Hay fever; Sinusitis; Eustachite; Otitis media (as part of combination therapy to reduce swelling of the nasopharyngeal mucosa); Preparing the patient for diagnostic manipulations in the nasal passages.
Special instructions
Do not exceed recommended doses, especially in children and the elderly. Long-term (more than 10 days) or excessive use of the drug can cause a “ricochet” effect (medicinal rhinitis) and/or atrophy of the nasal mucosa. Patients with long QT syndrome using xylometazoline may be at increased risk of developing severe ventricular arrhythmias. Benzalkonium chloride, which is part of the drug, can cause irritation of the nasal mucosa. The drug contains menthol and eucalyptol, which have a cooling effect on the nasal mucosa, giving a feeling of freshness.
Impact on the ability to drive vehicles and machinery
The drug does not affect the ability to drive vehicles or equipment. With long-term use in excess of recommended doses, the possibility of its systemic effect cannot be excluded. In such cases, caution should be exercised when driving or operating equipment.

Pregnancy
During pregnancy, the use of the drug is contraindicated. During breastfeeding, the drug should be used only after a thorough assessment of the risk-benefit ratio for the mother and child; the recommended dosage should not be exceeded.
Contraindications
Hypersensitivity to xylometazoline or any of the excipients of the drug; Arterial hypertension; Tachycardia; Severe atherosclerosis; Glaucoma; Hyperthyroidism; Atrophic rhinitis; Inflammatory diseases of the skin or mucous membrane of the nasal vestibule; Surgical interventions on the meninges (history); Condition after transsphenoidal hypophysectomy; Pregnancy; Use of monoamine oxidase inhibitors (MAO) (including 14 days after their discontinuation); Use of tricyclic or tetracyclic antidepressants; Children under 12 years old
With caution:
Diabetes mellitus; Severe cardiovascular diseases (including coronary heart disease, angina pectoris); Prostatic hyperplasia; Pheochromocytoma; Porphyria; Breastfeeding period; Hypersensitivity to adrenergic drugs, accompanied by insomnia, dizziness, arrhythmia, tremor, increased blood pressure; Patients with long QT syndrome.
Side Effects
From the immune system: very rarely – hypersensitivity reaction (angioedema, rash, itching). From the nervous system: often – headache. Mental disorders: rarely – insomnia, depression (with long-term use in high doses). On the part of the organ of vision: very rarely – impaired clarity of visual perception. From the side of the heart: rarely – palpitations; very rarely – tachycardia, arrhythmia. From the side of blood vessels: rarely – increased blood pressure. From the respiratory system, chest organs and mediastinum: often – irritation and/or dryness of the nasal mucosa, burning, tingling, sneezing, hypersecretion of the nasopharyngeal mucosa; rarely – after using the drug, increased swelling of the nasal mucosa (reactive hyperemia) and nosebleeds may be observed. Gastrointestinal disorders: often – nausea; rarely – vomiting. General disorders and reactions at the injection site: often – burning at the application site.
Interaction
Xylometazoline is contraindicated in patients currently receiving MAO inhibitors or who have received them within the previous 2 weeks. The simultaneous use of tri- or tetracyclic antidepressants and sympathomimetic drugs may lead to an increase in the sympathomimetic effect of xylometazoline, so this combination should be avoided.
Overdose
Xylometazoline, when administered topically in an excessive dose or accidentally ingested, can cause severe dizziness, increased sweating, a sharp decrease in body temperature, headache, bradycardia, increased blood pressure, respiratory depression, coma and convulsions. Following an increase in blood pressure, a sharp decrease may be observed.
Appropriate supportive measures should be taken if overdose is suspected, and in some cases immediate symptomatic treatment under medical supervision is indicated. These measures should include monitoring the patient for several hours. In case of severe poisoning with cardiac arrest, resuscitation efforts should last at least 1 hour.
