A combined antiepileptic drug.
Phenobarbital interacts with the “barbiturate” site of the benzodiazepine-GABAA-receptor complex, due to which it increases the sensitivity of GABAA-receptors to GABA, leads to opening of neuronal channels for chlorine ions, which leads to an increase of their entry into the cell. Reduces the excitability of neurons of the epileptogenic focus and distribution of nerve impulses. It shows antagonism to a number of excitatory mediators (glutamate and others).
It suppresses sensory zones of the cerebral cortex, reduces motor activity, suppresses cerebral functions, including the respiratory center. It reduces the tone of the smooth muscles of the gastrointestinal tract. It has anticonvulsant, sedative, hypnotic and antispasmodic effect.
Caffeine increases reflex excitability of the spinal cord, stimulates respiratory and vasomotor centers, stimulates metabolic processes in organs and tissues including muscular tissue and CNS.
Papaverine hydrochloride is an antispasmodic and has hypotensive effect. It reduces tone and relaxes smooth muscles of internal organs and blood vessels.
Calcium gluconate replenishes the lack of calcium ions necessary for the process of transmission of nerve impulses.
Bromisoval has sedative and mild sleeping pills.
Indications
– epilepsy.
Pharmacological effect
Combined antiepileptic drug.

Special instructions
In patients with mild hypercalciuria, decreased glomerular filtration, or a history of nephrolithiasis, the drug should be prescribed under the control of calcium levels in the urine, which is due to the presence of calcium gluconate in the drug.
Alcohol consumption is not recommended during therapy.
Impact on the ability to drive vehicles and operate machinery
During treatment, you should refrain from performing work that requires rapid psychomotor reactions (including driving). The drug should be stored in a place protected from light, out of reach of children, at a temperature not exceeding 25°C.

Excipients:
potato starch – 95 mg,
calcium stearate – 5 mg.
Contraindications
– severe liver and/or kidney failure;
— drug dependence (including history);
– hyperkinesis;
– myasthenia;
– severe anemia;
– porphyria;
– diabetes mellitus;
— hypofunction of the adrenal glands;
– hyperthyroidism;
– depression;
— broncho-obstructive diseases;
– active alcoholism;
– pregnancy;
– lactation period;
– hypersensitivity to any of the components of the drug.
This dosage form is not used in children under 7 years of age.
The drug should be used with caution in debilitated patients (high risk of paradoxical agitation, depression and confusion, even when prescribed in normal doses).
Side Effects
From the nervous system: dizziness, general weakness, ataxia, dysarthria, nystagmus, paradoxical reaction (especially in elderly and weakened patients – agitation), hallucinations, sleep disturbances.
From the digestive system: nausea, vomiting, constipation or diarrhea, increased activity of liver transaminases.
From the hematopoietic system: agranulocytosis, megaloblastic anemia, thrombocytopenia.
From the cardiovascular system: decrease or increase in blood pressure, tachycardia, arrhythmia (including extrasystole), AV block.
Other: allergic reactions, with prolonged use – drug dependence.
Interaction
Phenytoin and valproate increase the content of phenobarbital in the blood serum.
The effect of phenobarbital is reduced when taken simultaneously with reserpine, and increases when combined with amitriptyline, nialamide, diazepam, chlordiazepoxide.
Reduces the effectiveness of oral contraceptives and salicylates.
Reduces the blood levels of indirect anticoagulants, corticosteroids, griseofulvin, doxycycline, estrogens and other drugs metabolized in the liver via oxidation (accelerates their metabolism).
Enhances the effect of ethanol, neuroleptics, narcotic analgesics, muscle relaxants, sedatives and hypnotics.
Acetazolamide, by alkalinizing the urine, reduces the reabsorption of phenobarbital in the kidneys and weakens its effect.
Reduces the antibacterial activity of antibiotics and sulfonamides, the antifungal effect of griseofulvin.
Overdose
Symptoms: increased severity of side effects.
Treatment: gastric lavage, taking activated carbon, symptomatic therapy.
