Atypical antipsychotic (neuroleptic), it also has antidepressant and antiemetic effects. Neuroleptic effect is associated with antidopaminergic action. In central nervous system sulpiride blocks mainly dopaminergic receptors of limbic system, it has little effect on neostriate system, it has antipsychotic effect.
The peripheral action of sulpiride is based on inhibition of presynaptic receptors. With an increase in the amount of dopamine in the central nervous system is associated with improvement of mood, with a decrease – the development of depression.
The antipsychotic effect of sulpiride is seen in doses greater than 600 mg per day; in doses up to 600 mg per day, stimulant and antidepressant effects predominate. Sulpiride has no significant effect on adrenergic, cholinergic, seretonin, histamine and gamma-aminobutyric acid receptors.
Sulpiride stimulates prolactin secretion and has a central antiemetic effect (suppression of the vomiting center) due to blockade of dopamine D2-receptors of the vomiting center trigger zone.
Indications
Acute psychotic disorders in adults; chronic psychotic disorders in adults (schizophrenia, chronic non-schizophrenic delusions: paranoid delirium, chronic hallucinatory psychosis).

How to take, the dosage
Orally, with a small amount of liquid, regardless of meals.
Interaction
With concomitant use: with levodopa the efficacy of sulpiride decreases; with hypotensive drugs their hypotensive effect increases and the risk of orthostatic hypotension increases.
Special Instructions
With caution is used in patients with parkinsonism, the elderly. In patients with severe renal insufficiency, dose reduction or intermittent treatment is recommended.
In patients with epilepsy, a preliminary clinical and electrophysiological examination should be performed before initiating therapy, because sulpiride lowers the threshold of seizure readiness.
In case of hyperthermia, which is an element of MNS, sulpiride should be stopped immediately.
At all times during treatment, alcohol should be avoided.
With caution, use in children.
Influence on the ability to drive and operate machinery
During treatment, refrain from engaging in potentially hazardous activities requiring increased attention and rapid psychomotor reactions.

Contraindications
- Children under 18 years of age.
- Acute alcohol poisoning, sleeping pills, analgesics;
- hyperprolactinemia;
- galactosemia, glucose/galactose malabsorption syndrome or lactase deficiency;
- prolactin-dependent tumors (e.g., prolactinomas of the pituitary gland and breast cancer);
- pheochromocytoma;
- acute porphyria;
- current use of levodopa (see “Interaction with other drugs”).
- concomitant therapy with dopamine receptor agonists (cabergoline, quinagolide, ropinirole, rotigotine) (see section “Interaction with other medicinal products”).

Side effects
The adverse events that develop as a result of taking sulpiride are similar to the adverse events caused by other psychotropic drugs, but the frequency of their development is generally lower.
Overdose
Symptoms: dyskinesias (spasm of masticatory muscles, spastic torticollis), extrapyramidal disorders. In some cases – pronounced parkinsonism, coma.
Pregnancy use
In pregnancy and lactation, use with caution and in the lowest effective doses.
