Notropic agent. It has a positive effect on metabolic processes and blood circulation of the brain.
Enhances glucose utilization, improves metabolic processes, improves microcirculation in ischemic areas, inhibits the aggregation of activated platelets.
It has a protective effect on brain damage caused by hypoxia, intoxication and electroshock. It improves integrative activity of the brain. It has no sedative and psychostimulant effect.
Pharmacokinetics
In oral administration, it is quickly and almost completely absorbed from the gastrointestinal tract. Cmax in plasma is reached after about 30 min, in the CSF after 2-8 h. Estimated Vd is 0.6 l/kg. It does not bind with plasma proteins.
The drug is distributed in all organs and tissues, penetrates through the BBB and placental barrier. It selectively accumulates in cortical tissues, mainly in the frontal, parietal and occipital lobes, in the cerebellum and basal ganglia.
The T1/2 from plasma is 4-5 hours, from cerebrospinal fluid – 6-8 hours. It is excreted unchanged by kidneys. T1/2 is increased in renal insufficiency.
Indications
Brain trauma, in Alzheimer’s disease, in old age;
comatose states of vascular, traumatic or toxic genesis; treatment of withdrawal and psychoorganic syndrome in chronic alcoholism;
Learning disabilities in children not related to inadequate learning or family environment (as part of combination therapy);
Sickle cell anemia (as part of combination therapy).

How to take, the dosage
Ingestion.
With meals or on an empty stomach with fluids.
Symptomatic treatment of psychorganic syndrome: 2.4 – 4.8 g per day in 2-3 doses.
Treatment of dizziness and associated balance disorders: 2.4 – 4.8 g per day in 2-3 doses.
Treatment of cortical myoclonias:Start with a dose of 7.2 g per day, every 3-4 days the dose is increased by 4.8 g per day until a maximum dose of 24 g per day in 2 -3 doses is reached. Treatment is continued for the entire period of the disease. Every 6 months, attempts should be made to reduce the dose or to cancel the drug, gradually reducing the dose by 1.2 g per day every 2 days.
Treatment of sickle cell anemia:The daily prophylactic dose is 160 mg/kg body weight, divided into 4 equal doses.
Treatment of dyslexia in children (in combination with other treatments): the recommended daily dose for children from 8 years and adolescents is 3.2 g divided into 2 doses.
Interaction
Piracetam increases the effects of indirect anticoagulants, psychostimulants, central effects of thyroid hormones (anxiety, tremor, sleep disturbance, irritability, confusion).
The severity of extrapyramidal disorders is increased when co-administration of piracetam with neuroleptics.
Contraindications
Hemorrhagic stroke, severe renal failure (at CK< 20 ml/min), hypersensitivity to piracetam.
Side effects
Nervous system and senses:hyperkinesia, drowsiness, increased irritability, depression, agitation, asthenia, aggressive behavior, dizziness, sleep disturbance, headache, tremor, increased sexuality;
The digestive system: nausea, diarrhea, vomiting, abdominal pain;
Metabolism:increased body weight;
Circulatory system: exacerbation of coronary artery disease, arterial hypertension or hypotension;
Skin:hypersensitivity reactions.
Overdose
In case of overdose with piracetam side effects increase.
Treatment: gastric lavage, activated charcoal, symptomatic treatment; there is no specific antidote, hemodialysis is possible (effectiveness is 50-60%).
