Description

Slow calcium channel blocker (SCB), a 1,4-dihydropyridine derivative, has predominantly cerebrovasodilatory and antiischemic effects. It can prevent or eliminate vasospasm caused by various vasoactive substances (including serotonin, prostaglandins, histamine). Due to high lipophilicity, it penetrates through the blood-brain barrier.

Indications

Prevention and treatment of ischemic neurological disorders caused by cerebral vasospasm on the background of subarachnoid hemorrhage due to aneurysm rupture (after intravenous therapy with an infusion solution); marked disorders of brain function in the elderly (including reduced memory and ability to concentrate, emotional lability).

At the mouth, the tablets should be swallowed whole with a small amount of liquid, regardless of meals. Intervals between doses should be at least 4 hours. If there are no other prescriptions, the following dosing regimen is recommended:
1. Subarachnoid hemorrhage after aneurysm rupture. The recommended dose is 60 mg 6 times daily for 7 days (after infusion therapy with nimodipine solution).
2. cerebral dysfunction in elderly patients. The recommended dose is 30 mg 3 times a day.

Xenobiotics affecting CYP3A4 isoenzyme activity may alter the metabolic clearance of nimodipine.
Based on the experience of nifedipine use, it can be expected that rifampicin, by inducing liver enzyme activity, can accelerate nimodipine metabolism and the effectiveness of nimodipine is reduced when they are used simultaneously.

Administration of nimodipine to elderly patients with a large number of comorbidities, severe renal insufficiency (glomerular filtration rate less than 20 ml/min) and severe cardiovascular disease should be especially carefully justified. During and after therapy, such patients require regular medical monitoring (including neurological monitoring).

Pregnancy, lactation, individual intolerance to the components of the drug, marked liver dysfunction (for treatment of brain dysfunction), therapy with antiepileptic drugs, simultaneous use with rifampicin, age under 18 years.

Allergic reactions: skin itching, rash.
Cardiovascular system disorders: decreased blood pressure, bradycardia or tachycardia, arrhythmias, facial skin flushing, development or exacerbation of existing heart failure and myocardial ischemia (especially in patients with severe obstructive coronary artery disease).
Gastrointestinal tract disorders: nausea, diarrhea or constipation, flatulence, dry mouth, change of appetite (usually increase), gum hyperplasia (bleeding, painfulness, swelling), in rare cases – paralytic ileus.
Nervous system disorders: headache, dizziness, asthenia, extrapyramidal disorders (ataxia, mask-like face, muscle rigidity, tremor, difficulty swallowing),- symptoms of central nervous system agitation (insomnia, increased motor activity, agitation, aggressiveness), depression, excessive fatigue, sleepiness.
Hematopoietic system disorders: thrombocytopenia, agranulocytosis.
Respiratory system disorders: difficulty in breathing, cough, wheezing.
Motor system disorders: pain and swelling of joints.
Impact on laboratory parameters: increased activity of “hepatic” transaminases, alkaline phosphatase and gamma-glutamyltransferase, renal dysfunction with increased concentration of urea and/or creatinine in plasma.
Others: peripheral edema, increased sweating, decreased renal function (increased concentration of urea, hypercreatininemia), galactorrhea, weight gain.

In acute overdose of Nimopin, the following symptoms may occur: significant decrease in BP, tachycardia or bradycardia, epigastric pain, various neurological disorders.
