The active ingredient of Catena is gabapentin, a substance similar in structure to the neurotransmitter gamma-aminobutyric acid (GABA).
However, its mechanism of action differs from the effect of some other drugs that interact with GABA receptors, including barbiturates, GABA capture inhibitors, GABA agonists, valproate, GABA transaminase inhibitors, benzodiazepines and prodrug forms of GABA, because gabapentin has no GABAergic properties, does not affect the metabolism and capture of GABA.

for adults
Patients in this age group may use the drug compound if the above indications for use are identified. In some cases the drug is well tolerated and adverse reactions against the background of its use are extremely rare. The appearance of adverse reactions may be associated with the patient’s failure to comply with the basic norms and rules of taking the composition.
for children
The drug Katena may be used in pediatric practice if strict medical indications for admission are identified. Administration of the drug is prohibited for patients under the age of 6 years. Patients over the age of 6 years may use the drug as prescribed by the doctor to eliminate seizures. Persons over 12 years of age use the compound to treat epilepsy.
for pregnant and lactating women
Cautious use of the drug should be observed during breastfeeding and pregnancy. Pregnant women are prescribed the drug only in cases when the benefit of the treatment is several orders of magnitude higher than all the potential risks for the process of formation of the fetus in the womb. Breastfeeding should be stopped for the duration of therapy.

In combination with oral contraceptives, the composition does not affect their effectiveness.
The bioavailability of the drug may be slightly reduced when concomitantly taken with magnesium and aluminum-based antacids. To avoid an undesirable decrease in the therapeutic activity, a two-hour interval between doses should be observed.
The blood concentration may increase with morphine. Doses must be adjusted if these drugs are taken concomitantly.
The intensity of absorption of the active substance from the gastrointestinal tract is reduced when used concomitantly with antacids.
Directions for use
The capsule should be swallowed and drunk with water. The drug can be taken at any time, regardless of meals.
for adults
The single dose is 100 mg, and the frequency of administration is determined by the doctor. The allowable daily amount is calculated, taking into account individual tolerance and the body’s response to treatment. The dose is increased gradually.
The therapy of epilepsy is long-term, in most cases, for life.
In the treatment of patients over 12 years of age, 900 mg of gabapentinday is effective. At first, a minimum dose is prescribed, increasing it to the required dose every 3 days. The daily amount is usually divided into three doses. The maximum interval between doses of tablets should not be more than 12 hours. If it is necessary to replace the drug or cancel it completely, the dose should be reduced gradually.
Treatment of neuropathic pain begins with taking 300 mg of gabapentin three times a day. The dose is then adjusted by the physician and may be gradually increased to 3,600 mg. for children
Therapeutic doses for children over 12 years of age are the same as those prescribed for adult patients.
The appropriateness of the formulation for younger children is determined by the physician.
Pregnancy and lactation
Pregnancy and lactation should not use the formulation, because the active ingredients are absorbed in the breast milk and actively cross the placental barrier.

Special Instructions
Patients with diabetes should consult a physician to adjust the dosages of hypoglycemic medications.
Mental health needs to be monitored throughout the duration of Catena use, as it may cause depressive and suicidal moods.
It is extremely important that the dosing regimen prescribed by the physician is followed to prevent acute adverse reactions.
Abrupt discontinuation of the drug may cause seizure status.
If dose reduction is necessary, a physician should be consulted to work out an adequate regimen.
The patient taking the medication is not recommended to drive motor vehicles.
Contraindications
The main list of contraindications to the use of the composition includes the following restrictions:
- the age of the child under 12 years;
- lactose intolerance;
- lactase deficiency;
- pregnancy;
- lactation period;
- disorders of pancreatic function (including acute pancreatitis);
- malabsorption of glucose-galactose;
- different renal disorders.
If any contraindications are identified, taking the formulation is prohibited.
Side effects
The following reactions may occur while taking this medication:
- diplopia;
- leukopenia;
- depression;
- confusion;
- vibrations in BP level;
- myalgia and arthralgia;
- increased bone fragility;
- tremor;
- fever;
- disorder of movement coordination;
- insomnia or drowsiness;
- purpura;
- aggressiveness and restlessness;
- increased, weakened, or absent tendon reflexes;
- dizziness and headache;
- rhinitis;
- stomach and back pain;
- impotence;
- cutaneous allergic reactions;
- deterioration of vision;
- edema;
- dyspeptic reactions;
- increased appetite;
- emotional lability;
- cough.
The occurrence of these reactions is often seen as a reason to discontinue follow-up therapy.
Overdose
The following reactions may occur when doses are greatly overdosed:
- sleepiness;
- diarrhea;
- dizziness;
- disorders of speech and visual perception.
The administration of sorbents and gastric lavage are indicated to improve the patient’s well-being. Doses in excess of 20 times may be fatal.
The drug should be used under the supervision of a specialist. Uncontrolled use of the drug is unacceptable because of the high risks to the patient.
