Pharmacotherapeutic group:
Medication affecting metabolic processes (regulator of calcium and phosphorus metabolism).
ATS code [A12AX]
Pharmacological action:
A combined drug regulating calcium and phosphorus metabolism in the body. It reduces resorption (resorption) and increases bone density, making up for the lack of calcium. Vitamin D3 is essential for the mineralization of teeth.
Calcium is involved in the regulation of nerve conduction, muscle contractions and is a component of the blood clotting system.
Pharmacokinetic properties:
Vitamin D3 is absorbed in the small intestine. Calcium is absorbed in ionized form in the proximal small intestine through an active, D-dependent transport mechanism.
Indications
Therapeutic and prophylactic remedy for conditions of calcium and vitamin D3 deficiency in the body, associated with malnutrition or increased calcium and vitamin D3 requirement, including:
- Period of pregnancy and breastfeeding.
- Prevention and treatment of postmenopausal and senile osteoporosis, osteomalacia (as part of combination therapy).
Composition
Active ingredients:
Calcium carbonate is equivalent to 500 mg of calcium.
Vitamin D3 200 IU.
Excipients: corn starch, polyvinylpolypyrrolidone, magnesium stearate, hydroxypropylene cellulose, titanium dioxide, purified talc, polyethylene glycol, iron oxide brown, carnauba wax.
How to take, the dosage
Consult your doctor before use.
Ingestion. As a prophylactic agent, 1-2 tablets a day are prescribed for adults. To increase calcium absorption it is recommended to wash down the tablet with sour drinks (orange or lemon juice).
For course prevention of osteoporosis, 2 tablets a day, one in the morning and one in the evening, preferably with a meal.
For therapy of osteoporosis, the course and dosage according to the doctor’s recommendation.
Interaction
- Vitamin D3 activity may decrease with concomitant use of phenytoin or barbiturates;
- Concomitant treatment with cardiac glycosides requires ECG and clinical monitoring, as calcium drugs enhance the effects of cardiac glycosides.
- Calcium preparations increase the effect of cardiac glycosides
- The drug slows down absorption of tetracycline antibiotics, acetylsalicylic acid and other salicylates, sulfonamides, beta-adrenoblockers and indirect anticoagulants from gastrointestinal tract. In this regard, it is recommended to use 2 hours or 2 hours after taking other drugs.
- Colestyramine, laxatives based on mineral or vegetable oil may reduce absorption of vitamin D3.
- Concomitant use of diuretics from the thiazide group increases the risk of hypercalcemia.
Special Instructions
- Cautiously used in persons with renal insufficiency;
- At all times during treatment, urinary calcium excretion and plasma calcium and creatinine concentrations should be monitored (if calciuria exceeds 7.5 mmol/day (300 mg/day), the dose should be reduced or discontinued);
- In order to avoid overdose, additional vitamin D3 intake from other sources should be considered. Do not use simultaneously with vitamin complexes containing calcium and vitamin D3;
- The elderly have a calcium requirement of 1.5 g/day, vitamin D3 500-1000 IU/day.
.
Contraindications
- High sensitivity to the drug components.
- Hypercalcemia (increased concentration of calcium in the blood – in persons with increased parathyroid function)
- Hypercalciuria (increased concentration of calcium in the urine).
- Urolithiasis (formation of calcium stones).
- Decalcifying tumors (myeloma, bone metastases, sarcoidosis).
- Osteoporosis due to immobilization.
- Pulmonary tuberculosis (active form).
- childhood.
Overdose
Symptoms of acute and chronic overdose are hypercalcemia or/and hypercalciuria, which are expressed as anorexia, nausea, vomiting, dizziness, weakness, headache. In these cases, it is recommended to stop taking the drug. Large amounts of fluids should be introduced into the body and a diet with a limited amount of calcium should be adhered to. If symptoms of overdose appear, it is necessary to consult a doctor. Overdose during pregnancy can lead to impaired mental and physical development of the child. Ca2+ and vitamin D3 penetrate into breast milk.
Pregnancy use
The need for calcium and vitamin D during pregnancy and lactation is increased. When taking the drug it is necessary to take into account the additional intake of calcium and vitamin D3 from other sources and to carry out constant medical monitoring.
In order to avoid toxic effects on the fetus, it is not recommended to take more than 1500 mg of calcium and 600 IU of vitamin D per day. Calcium and vitamin D pass into breast milk.
