Pharmacodynamics
Direct anticoagulant, has anti-inflammatory, antiproliferative, anti-edema and analgesic effect.
Reduces platelet aggregation, binds antithrombin III, preventing the conversion of prothrombin to thrombin. Inhibits thrombin activity.
Reduces hyaluronidase activity, increases blood fibrinolytic properties. Trombless® improves microcirculation and activates tissue exchange, thus accelerating the resorption of hematomas and
clots and finally restores venous patency, clinically it is accompanied by a pronounced analgesic and anti-inflammatory effect.
Pharmacokinetics
A small amount of heparin is absorbed from the surface of the skin into the systemic blood flow.
Maximal concentration of the drug in blood is observed 8 hours after application.
Heparin excretion mainly occurs through the kidneys, the half-life is 12 hours.
Indications
Swelling, Varicose veins, Swelling after injuries and operations, Sprains and strains, Bruises, Hematomas, Heavy and painful legs, Thrombophlebitis, Inflamed tendons
- Migrating phlebitis in CVI;
- thrombophlebitis of superficial limb veins and varicose ulcers;
- subcutaneous hematomas
- postinjection phlebitis and phlebitis after surgical interventions on the veins
- injuries and contusions of tendons and muscle tissue
- local soft tissue edema.

Composition
1 g of gel contains:
Active ingredient:
Sodium heparin – 1000 ME (0,00833 g with sodium heparin activity 120 ME/mg);
Excipients:
Ethanol (ethyl alcohol) – 0.24 g,
Methylparahydroxybenzoate (nipagin, methylparaben) – 0.0012 g,
Propylparahydroxybenzoate (nipazole, propylparaben) – 0,0003 g, diethanolamine (2,2-diiminoethanol) – up to 0,014 g to obtain gel with pH from 7.5 to 8.7,
Carbomer (carbopol) – up to 0.015 g,
Water (purified water) – up to 1 g.
How to take, the dosage
Trombless gel is used externally. The gel is 3-10 cm (depending on the area of the affected area) lightly rubbed into the skin 1-3 times a day.
The treatment is applied daily for 4 to 7 days. The course of treatment is determined by the doctor.
In the treatment of hemorrhoidal nodules the instructions for use of Trompless prescribes the application of the drug on a cotton swab which is applied to the affected area.
Interaction
Concomitant use of the gel with indirect anticoagulants may cause prolongation of prothrombin time.
Gel is not administered topically simultaneously with nonsteroidal anti-inflammatory drugs, tetracyclines, antihistamines.
Special Instructions
The drug should not be used in case of bleeding, as well as on open wounds, mucous membranes and purulent processes.
Contraindications
Hypersensitivity to the drug components.
Ulcerative-necrotic, purulent processes on the skin; traumatic skin integrity disorders; increased susceptibility to bleeding, thrombocytopenia.
Side effects
Long-term use of Trompless may cause local reactions in the form of skin hyperemia and allergic reactions.

Overdose
Overdose is unlikely because systemic absorption is minimal.
Pregnancy use
The percutaneous route of administration is considered the safest – the gel acts locally and provides a concentration of the drug at the lesion site.
There is virtually no overall resorptive effect, and the dose can be reduced if necessary.
These points allow the drug to be used in pregnancy with varicose veins.
