Amlorus is a dihydropyridine derivative, a II generation “slow” calcium channel blocker with antianginal and hypotensive effects. Binding to dihydropyridine receptors, it blocks calcium channels, reduces transmembrane transfer of calcium ions into the cell (more in vascular smooth muscle cells than in cardiomyocytes).
Indications
Arterial hypertension (monotherapy or in combination with other antihypertensive agents: diuretics, beta-adrenoblockers, ACE (angiotensin-converting enzyme) inhibitors).

How to take, the dosage
Amlorus is taken orally.
In arterial hypertension and angina pectoris the initial dose is 5 mg once daily. If necessary, the dose may be increased up to 10 mg once a day.
In arterial hypertension, the maintenance dose may be 5 mg per day. In vasospastic angina (Prinzmetal’s angina), 5-10 mg once daily.
Dose changes are not required when concomitantly administered with thiazide diuretics, beta-adrenoblockers and angiotensin-converting enzyme (ACE) inhibitors. No dose changes are required in patients with renal insufficiency, in patients with hepatic impairment and elderly patients – the initial dose in arterial hypertension may be 2.5 mg.
Interaction
Microsomal oxidation inhibitors increase the plasma concentration of amlodipine, increasing the risk of side effects, while microsomal liver enzyme inducers decrease it.
Special Instructions
During treatment it is necessary to control body weight and sodium intake, prescribing an appropriate diet.
Contraindications
- high sensitivity to amlodipine and other dihydropyridine derivatives;
- serious arterial hypotension;
- collapse, cardiogenic shock;
- unstable angina (except for Prinzmetal angina);
- pregnancy and lactation;
- age below 18 years (efficacy and safety not established).
With caution: Impaired liver function, sinus node weakness syndrome (marked bradycardia, tachycardia), decompensated chronic heart failure, mild to moderate degree of arterial hypotension, aortic stenosis, mitral stenosis, hypertrophic obstructive cardiomyopathy, acute myocardial infarction (and within 1 month after), diabetes, lipid profile disorders, elderly age.
Side effects
Cardiovascular system: palpitations, dyspnea, marked decrease in blood pressure, syncope, vasculitis, edema (swollen ankles and feet), “flushes” of blood to the face, rarely – rhythm disturbances (bradycardia, ventricular tachycardia, atrial flutter), chest pain, orthostatic hypotension, very rare – the development or worsening of heart failure, extrasystole, migraine.
Overdose
Symptoms: marked decrease in blood pressure, tachycardia, excessive peripheral vasodilation.
