Tolperizone is a central myorelaxant, the mechanism of action of which is not fully understood. It is known that the drug has a high affinity (binding strength) to the nervous tissue, the highest concentrations are achieved in the brain stem, in the peripheral nervous system and the spinal cord.
Indications
According to the instructions, Tolperisone is used to treat stroke-related spasticity in adults. The use of the drug is also possible in the following cases:
- extrapyramidal disorders (atherosclerotic and post-encephalitic parkinsonism);
- hypertonicity combined with another type of muscle tone disorder;
- sequelae of vascular innervation disorders (intermittent angioneurotic dysbasia, acrocyanosis);
- diseases accompanied by muscle spasm, rigidity, dystonia;
- obliterating arterial diseases: obliterating atherosclerosis of limb vessels, Buerger’s disease (obliterating thrombangiitis), diabetic angiopathy, Raynaud’s syndrome;
- childhood spastic paralysis (Little’s disease);
- Encephalopathy of vascular genesis;
- Epilepsy;
- postthrombotic disorders of lymph circulation and venous circulation, trophic ulcers of the shin.

How to take, the dosage
Tolperizone tablets should be taken orally after a meal: swallow them whole without breaking or chewing, and drink plenty of water. When taking the drug on an empty stomach, its bioavailability is reduced. The initial dose is 50 mg 3 times a day, then it is gradually increased to 150 mg, keeping three doses a day.
Interaction
Tolperisone in concomitant use may increase the blood content of drugs that are metabolized primarily with the participation of CYP2D6 isoenzyme, such as desipramine, venlafaxine, metoprolol, tolterodine, perphenazine, dextromethorphan, thioridazine, nebivololol, atomoxetine.
Special Instructions
Frequent side effects of the drug are hypersensitivity reactions, which can range from mild cutaneous to severe systemic, including anaphylactic shock.
Contraindications
- Age under 18 years of age;
- glucose-galactose malabsorption, lactase deficiency, lactose intolerance;
- myasthenia gravis;
- breastfeeding;
- high sensitivity to any of the ingredients of the drug.

Side effects
According to the data of clinical use of the drug in more than 12,000 patients, the most frequently noted are general disorders, gastrointestinal and neurological phenomena, as well as disorders of the skin and subcutaneous tissues.
The development of hypersensitivity reactions was reported in the post-registration period, with 50-60% of all findings. In the predominant case there were non-serious side effects. Severe, life-threatening allergic reactions were very rarely reported.
According to the recommendations of the World Health Organization, side effects that occur during therapy with tolperizone are classified as follows: very frequent – ≥ 1/10, frequent – ≥ 1/100 to < 1/10, infrequent – ≥ 1/1000 to < 1/100, rare – ≥ 1/10 000 to < 1/1000, very rare – < 1/10 000, including individual cases, unknown frequency – based on the available data, it is impossible to calculate the frequency of adverse reactions.
Tolperizone may cause the following disorders of various organs and body systems:
- nervous system: infrequently – drowsiness, headache, dizziness; rarely – paresthesia, malaise, seizures, tremors, attention deficit disorder, epilepsy, lethargy;
- psychic: infrequent – sleep disturbances, insomnia; rarely – depression, feeling of weakness; very rarely – confusion;
- metabolism: infrequent – anorexia; very rare – polydipsia;
- cardiovascular system: infrequent – decreased blood pressure; rare – palpitations, tachycardia, flushes to the face, angina pectoris; very rare – bradycardia;
- liver and biliary tract: rare – moderate liver failure;
- liver and urinary tract: rarely – proteinuria, enuresis;
- respiratory system: rarely – nasal bleeding, shortness of breath, tachypnea;
- blood and lymphatic system: very rarely – lymphadenopathy, anemia;
- gastrointestinal tract: infrequent – dry mouth, abdominal discomfort, diarrhea, nausea, dyspepsia; rarely – flatulence, constipation, epigastric pain, vomiting;
- skeletal-muscular system and connective tissue: infrequent – pain in extremities, weakness and pain in muscles; rare – feeling of discomfort in extremities; very rare – osteopenia;
- immune system: rare – anaphylactic and hypersensitivity reactions (Quincke’s edema, including lips and face); very rare – anaphylactic shock;
- skin and subcutaneous tissue: rarely – hyperhidrosis, skin rash, allergic dermatitis, urticaria, skin itching;
- hearing and visual organs: rarely – tinnitus, vertigo, decreased visual acuity;
- laboratory parameters: rarely – leukocytosis, thrombocytopenia, hyperbilirubinemia, impaired liver function; very rarely – hypercreatininemia;
- others: infrequently – malaise, fatigue, asthenia; rarely – thirst, feeling warm, irritability, feeling intoxicated; very rarely – discomfort in the chest.

Overdose
There are few reported cases of overdose. In preclinical studies of acute toxicity of tolperizone high doses have resulted in tonic-clonic seizures, ataxia, dyspnea and respiratory paralysis.
