Thioridazine, the active ingredient in Thioril, is a piperidine derivative of phenothiazine. It has antipsychotic, tranquilizing, antidepressant, antipruritic, alpha-adrenoblocking and m-cholinoblocking effects.
In small doses the substance reduces anxiety and tension, has an anxiolytic effect, in large doses it shows neuroleptic properties. When administered in medium therapeutic doses, it does not cause somnolence and extrapyramidal disorders. Besides, thioridazine has weak antiemetic and moderate hypotensive effects. The central antiemetic effect of the drug is explained by inhibition or blockade of dopamine Og-receptors in the chemoreceptor trigger zone of the cerebellum, peripheral – by blockade of the vagus nerve in the gastrointestinal tract (GIT).
The antipsychotic (neuroleptic) action of Tioril is explained by blockade of postsynaptic dopaminergic receptors in the mesolimbic structures of the brain. Thioridazine inhibits release of hypothalamic and pituitary hormones (but pituitary prolactin release increases due to blockade of dopamine receptors), has alpha-adrenoblocking action.

Indications
Schizophrenia, psychotic disorders accompanied by hyperactivity and psychomotor agitation; behavioral disorders associated with psychotic disorders and/or neurological diseases; moderate depressive disorders; neuroses accompanied by anxiety, agitation, tension, fear.
Pharmacological effect

In small doses, the substance reduces feelings of anxiety and tension, has an anxiolytic effect, in large doses it exhibits neuroleptic properties. When taken in average therapeutic doses, it does not cause drowsiness or extrapyramidal disorders. In addition, thioridazine has weak antiemetic and moderate hypotensive effects. The central antiemetic effect of the drug is explained by inhibition or blockade of dopamine O2 receptors in the chemoreceptor trigger zone of the cerebellum, and the peripheral effect by blockade of the vagus nerve in the gastrointestinal tract (GIT).
The antipsychotic (neuroleptic) effect of Thioril is explained by the blockade of postsynaptic dopaminergic receptors in the mesolimbic structures of the brain. Thioridazine suppresses the release of hormones from the hypothalamus and pituitary gland (however, the release of prolactin by the pituitary gland increases due to the blockade of dopamine receptors), and has an alpha-adrenergic blocking effect.
Special instructions
Use with caution in patients with cardiac arrhythmias, heart disease, severe respiratory diseases, renal failure, Parkinson’s disease, angle-closure glaucoma, prostatic hypertrophy, myasthenia gravis, epilepsy, pheochromocytoma.
During the treatment period, avoid drinking alcohol.
Patients with liver disease require regular monitoring of liver function. In elderly patients, debilitated patients or those suffering from kidney or liver disease, it is recommended to prescribe a low initial dose with a gradual increase in dose.

During the treatment period, you should refrain from potentially hazardous activities that require increased attention and rapid psychomotor reactions.

Contraindications
Severe depressive states, comatose states, severe depression of the central nervous system, a history of blood diseases, children under 2 years of age, severe diseases of the cardiovascular system, liver failure, hypersensitivity to thioridazine and other phenothiazine derivatives.
Side Effects
From the side of the central nervous system: possible drowsiness, confusion, anxiety, akathisia, headache, visual disturbances; in isolated cases – neuroleptic malignant syndrome; convulsions, because Thioril may lower the seizure threshold. With prolonged use, extrapyramidal disorders are possible. In high doses, it causes pigmentary retinopathy more often than other phenothiazines. In very rare cases, tardive dyskinesia may occur with long-term use.
Interaction
With simultaneous use of Thioril with other drugs that have a depressant effect on the central nervous system, as well as ethanol and ethanol-containing drugs, increased depression of the central nervous system is possible; with levodopa – decreased antiparkinsonian effect; with lithium preparations – possible neurotoxic effect.
Overdose
Symptoms: increased drowsiness, confusion, urinary retention, depression of the respiratory center, arterial hypotension, disorientation, areflexia, hyperreflexia, convulsions, arrhythmia.
