Tyocetam belongs to the group of cerebroactive agents and shows nootropic anti-ischemic and antioxidant properties.

The pharmacological effect of the drug is due to the mutual potentiation of thiotriazolin and piracetam. Piracetam is a cyclic derivative of GABA, a nootropic agent; it acts directly on the brain, improving such functions as learning, memory, attention and consciousness without having sedative or psychostimulant effects.

At the neuronal level, piracetam facilitates various types of synaptic transmission with a predominant effect on the density and activity of postsynaptic receptors.

The hemorheological effects of piracetam are related to its effect on red blood cells, platelets and the vascular wall. Thiotriazolin reacts with reactive oxygen species and lipid radicals due to the pronounced reductive properties of the thiol group and prevents the formation of reactive oxygen species by reactivating antiradical enzymes: superoxide dismutase, catalase and glutathione peroxidase.

Thiocetam improves integrative and cognitive activity of the brain, facilitates the learning process eliminates amnesia, improves short- and long-term memory.

How to take, the dosage

The drug is administered intravenously, intramuscularly only if oral administration is impossible.

In chronic disorders of cerebral circulation and in the recovery period after ischemic stroke 20-30 ml of the drug, pre-diluted in 100-150 ml of 0.9% sodium chloride solution, are administered by intravenous drip once a day.

The course of treatment is 2 weeks, subsequently changing to a tablet form.

In diabetic encephalopathy 5 ml is administered intravenously 1 time per day for 10 days with further administration of the tablet form.

Interaction

Thiocetam should not be administered simultaneously with drugs that have an acidic pH value.

The presence of piracetam increases the effect of antidepressants and antianginal drugs. When concomitant administration with thyroid hormones, confusion, irritability and sleep disorders are possible.

Piracetam does not inhibit cytochrome P450 isoenzymes. Metabolic interaction with other drugs is unlikely

Co-administration with alcohol had no effect on the serum concentration level of piracetam; the serum concentration of ethanol was not altered when 1.6 g of piracetam was taken.

Special Instructions

In long-term therapy in elderly patients, regular monitoring of renal function is recommended; if necessary, the dose should be adjusted depending on creatinine clearance.

Impact on the ability to drive vehicles and operate machines and mechanisms

At the time of treatment it is necessary to refrain from driving vehicles and engaging in potentially dangerous activities requiring increased concentration and rapid psychomotor reactions (driving, etc.).

Contraindications

Side effects

Nervous system disorders: motor disinhibition, irritability, somnolence, depression, asthenia, headache, dizziness, tinnitus, insomnia, mental agitation, balance disorder, ataxia, worsening of epilepsy, anxiety, hallucinations, confusion.

Gastrointestinal disorders: nausea, vomiting, diarrhea, abdominal pain (including gastralgia) and bloating, dry mouth.

Cardiovascular system disorders: tachycardia, increased blood pressure, pain in the heart region, arrhythmias; in elderly people coronary insufficiency may worsen.

Allergic reactions: hypersensitivity, anaphylactic reactions, including dermatitis, pruritus, urticaria and angioedema, fever.

Respiratory system disorders: dyspnea, episodes of choking.

Metabolic disorders: weight gain.

Hearing disorders: vertigo.

Overdose

Symptoms: occurrence or increase in the severity of dose-dependent side effects of the drug.

Treatment: drug withdrawal, induction of vomiting, gastric lavage, symptomatic therapy.

Pregnancy use

There is no experience of using the drug during pregnancy or lactation; therefore, the drug should not be prescribed to this category of women.

There is no experience of using the drug for treatment of children, therefore the drug is contraindicated for treatment of children under 18 years of age.
