Progesterone is a gestagen. Hormone of the corpus luteum. Causes the uterine mucosa to move from the proliferation phase caused by follicular hormone to the secretory phase and, after fertilization, promotes its transition to the state necessary for the development of the fertilized egg.
It reduces the excitability and contractility of the uterine and fallopian tube muscles and stimulates the development of mammary gland terminal elements. It does not have androgenic activity.
Progesterone has a weak effect on protein metabolism, promotes fat deposition and glucose accumulation in the liver and reduces sodium reabsorption in the renal tubules.
Progesterone has a blocking effect on the secretion of hypothalamic release factors LH and FSH, inhibits the formation of gonadotropic hormones in the pituitary gland and inhibits ovulation.
Pharmacokinetics
After injection, it is absorbed rapidly and almost completely. Metabolized in the liver, most metabolites form conjugates with glucuronic and sulfuric acids.
The T1/2 from plasma is a few minutes. It is excreted in the urine – 50-60% and in the feces – more than 10%. The amount of metabolites that are excreted in the urine varies depending on the phase of the corpus luteum.

Dose, frequency and duration of treatment are determined according to the scheme depending on the indication and clinical situation.
Interaction
Reduces the intensity of the effect of drugs that stimulate uterine smooth muscles, anabolic steroids, gonadotropic hormones of the anterior pituitary lobe.
Decreases the lactogenic effect of oxytocin.
It increases the effect of diuretics, antihypertensive drugs, immunosuppressants.
In concomitant use with barbiturates there is a decrease in the effect of progesterone.
Contraindications
Severe liver or renal dysfunction, hepatitis, thromboembolic diseases, vaginal bleeding of undetermined genesis, breast carcinoma (except in cases in which progesterone is used as part of combined therapy of this disease).
Side effects
Cardiovascular system: increase in BP.
Metabolism disorders: edema.
In the digestive system: liver dysfunction, jaundice; rarely (with long-term use) – nausea, vomiting.
Endocrine system disorders: rare (in long-term use) – decreased libido, weight gain, pain and tension in the mammary glands, irregular uterine bleeding.
CNS disorders: rarely (with long-term use) – headache, depression.
