Special Instructions

Before starting cilostazol therapy, the possibility of prescribing other treatment options, such as surgical revascularization or conservative therapy, should be evaluated.

Contraindications

• High sensitivity to cilostazol or any other component of the drug;
• Severe renal failure (creatinine clearance ≤25 ml/min);
• Moderate or severe hepatic insufficiency;
• Chronic heart failure;
• Predisposition to bleeding (e.g., acute gastric or duodenal ulcer, recent hemorrhagic stroke [within the last 6 months], proliferative diabetic retinopathy, poorly controlled arterial hypertension);
• Ventricular tachycardia, ventricular fibrillation, or polytopic ventricular extrasystole in history (regardless of presence or absence of adequate antiarrhythmic therapy);
• History of prolonged QT interval on ECG;
• Severe tachyarrhythmia;
• Unstable angina or myocardial infarction within the last 6 months;
• Invasive coronary artery surgery within the last 6 months;
• Simultaneous use of two or more antiplatelet or anticoagulant medications (e.g., acetylsalicylic acid, clopidogrel, heparin, warfarin, acenocoumarol, dabigatran, rivaroxaban, or apixaban);
• Simultaneous use of potent CYPZA4 or CYP2C19 inhibitors (e.g., cimetidine, diltiazem, erythromycin, ketoconazole, lansoprazole, omeprazole, and HIV-1 protease inhibitors);
• Pregnancy;
• Breastfeeding period;
• Age under 18 years of age (safety and efficacy not studied).

With caution

• Mild hepatic insufficiency;
• Cronic coronary heart disease (in particular, stable angina pectoris);
• Atrial or ventricular extrasystole, atrial fibrillation, and flutter;
• Diabetes mellitus;
Simultaneous use of blood pressure lowering medications;
• Simultaneous use of blood clotting reducing medications;
• Concomitant use of CYPCA4 or CYP2C19 substrates (e.g., cisapride, midazolam, nifedipine, verapamil);
• Elderly age.

Side effects

The most commonly reported side effects in clinical trials were headache (>30%), diarrhea, and stool disorders (>15% each). These side effects were mild to moderate in intensity.

Overdose

Symptoms
Information about acute cilostazol overdose in humans is limited. Expected symptoms of overdose: pronounced headache, diarrhea, tachycardia, arterial hypotension, cardiac arrhythmia.
