Pirantel, suspension 250 mg/5 ml 15 ml

Pirantel is an anthelmintic.

Blocks neuromuscular transmission in susceptible helminthes.

Pharmacodynamics

Acts on helminths in the early phase of development and mature forms, has no effect on larvae in the migration stage. It is active against Enterobius vermicularis, Ascaris lumbricoides, Ancylostoma duodenale, Necator americanus, Trichostrongylus orientalis, Trichostrongylus colubriformis.

Pharmacokinetics

It is practically not absorbed in the gastrointestinal tract. After oral administration at a single dose of 10 mg/kg body weight Cmax is 0.005-0.13 mcg/ml and is reached within 1-3 hours.

Partially metabolized in the liver to N-methyl-1,3-propanediamine. It is excreted in the faeces (50% of the taken dose – unchanged) and in the urine (about 7% – unchanged and as a metabolite).

Indications

Enterobiasis, ascariasis, ankylostomidiasis, necatoriasis.

How to take, the dosage

In case of ascariasis and enterobiasis, in mixed infestations with these helminthes – orally, once, after breakfast at the rate of 10 mg/kg.
Adults and children over 12 years – 750 mg, if body weight over 75 kg – 1 g.

In children from 6 months to 2 years – 125 mg, 2 – 6 years – 250 mg, 6 – 12 years – 500 mg. In order to avoid self-infection it is allowed to repeat the course in 3 weeks after the first intake.

In case of ankylostomiasis, combined necatoriasis and ascariasis or other combined helminth infestations the dose is 10 mg/kg/day for 3 days or 20 mg/kg/day for 2 days.

In case of isolated ascariasis – 5 mg/kg once.

Interaction

Should not be administered simultaneously with piperazine (weakening of action).

Pirantel increases the concentration of theophylline in plasma when used concomitantly.

Special Instructions

No laxatives are prescribed after pyrantel administration.

In case of enterobiasis all persons living together should be treated at the same time. After completion of treatment it is necessary to conduct a control examination of feces for the presence of parasite eggs.

Careful hygiene (cleaning of living rooms and cleaning of clothing to destroy the helminth eggs serves as a prevention of reinfection).

Side effects

Nervous system and sensory organs: rarely – headache, dizziness, drowsiness, insomnia, weakness; in some cases – hearing impairment, hallucinations, confusion, paresthesias.

Gastrointestinal system disorders: nausea, vomiting, diarrhea, stomach pain; rarely – increased liver transaminase activity.

Other: skin rash, fever.

Pregnancy use

Pregnancy and breast-feeding can be used only under the strict supervision of a doctor.
