Pantogam active is a nootropic, anticonvulsant.
Pharmacodynamics
The action of Pantogam activity® is due to the presence in its structure of gamma-amino butyric acid, which directly affects the GABAB receptor-channel complex. Pantogam active® is a racemic mixture of equal amounts of the R-form of hopantenic acid and its S-isomer.
The presence of the isomer improves the transport and interaction of the drug with the GABAA receptor. Pantogam active® has more pronounced nootropic and anticonvulsant activity than the preparations of the first generation of propanthenic acid.
Pantogam active® increases resistance of the brain to hypoxia and toxic substances, stimulates anabolic processes in neurons, combines moderate sedative action with a mild stimulating effect.
It has anti-asthenic and mild anti-anxiety effects. It reduces motor excitability and regulates behavior. It activates mental activity and ability to work. Causes inhibition of pathologically increased bladder reflex and detrusor tone.
Pharmacokinetics
It is rapidly absorbed from the gastrointestinal tract. It penetrates through the HEB. It is not metabolized and eliminated unchanged in 48 hours: about 70% of the administered dose – in the urine, about 30% – in the faeces.
Indications
Cognitive impairment in organic brain lesions (including consequences of neuroinfections and traumatic brain injuries) and neurotic disorders;
cerebrovascular insufficiency caused by atherosclerotic changes in cerebral vessels;
extrapyramidal hyperkinesis (myoclonus epilepsy, Huntington’s chorea, hepatolenticular degeneration, Parkinson’s disease, etc.), treatment of extrapyramidal syndrome caused by taking antipsychotics;
epilepsy (together with anticonvulsants);
psycho-emotional overload, decreased mental and physical performance, to improve concentration and memory;
neurogenic urinary disorders (pollakiuria, urgency, imperative urinary incontinence, enuresis);
schizophrenia (as part of complex therapy).

Active ingredient:
rac-hopantenic acid (D,L-hopantenic acid);
Excipients:
microcrystalline cellulose;

Contraindications
Hypersensitivity;
acute severe kidney disease;
pregnancy (first trimester);
breastfeeding period;
the patient’s age is under 18 years (there are no clinical studies on the use of the drug at an earlier age).
Side Effects
In rare cases, allergic reactions (rhinitis, conjunctivitis, skin rashes) are possible.
In this case, reduce the dose or discontinue the drug.
Sleep disturbances or drowsiness, noise in the head – these symptoms are usually short-lived and do not require discontinuation of the drug.
Interaction
Prolongs the effect of barbiturates, enhances the effects of anticonvulsants, prevents the side effects of phenobarbital, carbamazepine, and neuroleptics.
The effect of Pantogam active® is enhanced when combined with glycine and etidronic acid.
Potentiates the effect of local anesthetics (procaine).
Overdose
Symptoms: increased side effects.
Treatment: administration of activated carbon, gastric lavage, symptomatic therapy.
