Pharmacodynamics

The important intracellular cations K⁺ and Mg²⁺ play a key role in the function of numerous enzymes, the formation of bonds between macromolecules and intracellular structures, and the mechanism of muscle contractility. The intra- and extracellular ratio of potassium, calcium, sodium and magnesium ions influences myocardial contractility.

Exogenous aspartate acts as a conductor of ions: it has a high affinity for cells, due to the slight dissociation of its salts, ions in the form of complex compounds penetrate the cell. Magnesium and potassium aspartates improve myocardial metabolism. Magnesium/potassium deficiency predisposes to the development of hypertension, coronary artery atherosclerosis, arrhythmias and metabolic changes in the myocardium.

Pharmacokinetics

Absorption is high. It is excreted by the kidneys.

Indications

Heart failure, myocardial infarction, cardiac rhythm disorders (mainly ventricular arrhythmias), adjunctive therapy with cardiac glycosides, hypokalemia, hypomagnesemia, insufficient intake of magnesium and potassium ions in the body with their deficiency in the diet.

Composition

Active substances:

Potassium asparaginate (K+ = 103.3 mg) 452 mg,

Magnesium asparaginate (Mg++ = 33.7 mg) 400 mg;

How to take, the dosage

Intravenously, dropwise, as a slow drip infusion (20 drops/minute).

The contents of 1-2 ampoules are dissolved in 50-100 ml of 5% dextrose (glucose) solution.

If necessary, the administration can be repeated after 4-6 hours.

The drug is suitable for combined therapy.

Interaction

Inhibits absorption of oral forms of tetracycline, iron salts and sodium fluoride (requires a 3-hour break between doses).

Because of potassium presence in the drug the risk of hyperkalemia increases (it is necessary to control the level of potassium in plasma) in combination with ACE inhibitors, beta-adrenoblockers, cyclosporine, potassium-saving diuretics, heparin and NSAIDs; inhibitory influence on intestinal peristalsis increases. In the development of hyperkalemia, the effect of foxglove drugs may be reduced.

Due to the presence of magnesium in the drug, neuromuscular blockade is increased when combined with antidepolarizing myorelaxants, CNS depression – when combined with agents for anesthesia.

Decreases the effectiveness of neomycin, polymyxin B, tetracycline and streptomycin. Calcium preparations reduce efficacy, and calcitriol contributes to elevated plasma magnesium levels.

Special Instructions

Panangin should be used with caution in patients with an increased risk of hyperkalemia.

Contraindications

Hypersensitivity, acute and chronic renal failure, Addison’s disease, anuria, AV block of degree II-III, cardiogenic shock, severe arterial hypotension, hyperkalemia, hypermagnesemia, severe myasthenia, oliguria, hemolysis, amino acid metabolism disorders.

Side effects

Nervous system and sensory organs: increased fatigue, hyporeflexia, dizziness, fever, paresthesia.

Cardiovascular system and blood (hematopoiesis, hemostasis): BP decrease, ECG changes, myocardial conduction and cardiac rhythm disorders, phlebitis, vasospasm, vasodilation, venous thrombosis.

Respiratory system: dyspnea.

Gastrointestinal system disorders: diarrhea, nausea, vomiting, dry mouth, rarely – abdominal pain, bleeding and ulceration of the intestine and gastric mucosa, perforation of the intestinal wall, flatulence, esophageal and duodenal ulcer.

Metabolism: hyperkalemia.

Musculoskeletal system: muscle weakness.

Skin: itching, increased sweating.

Overdose

Symptoms: conduction disorders (especially with previous pathology of the cardiac conduction system).

Treatment: intravenous administration of calcium chloride; if necessary – hemodialysis and peritoneal dialysis.
