A combined antiepileptic drug.
Phenobarbital interacts with the “barbiturate” site of the benzodiazepine-GABAA-receptor complex, due to which it increases the sensitivity of GABAA-receptors to GABA, leads to opening of neuronal channels for chlorine ions, which leads to an increase of their entry into the cell. It reduces the excitability of neurons of the epileptogenic focus and distribution of nerve impulses.
It shows antagonism to a number of excitatory mediators (glutamate and others). Suppresses sensory areas of the cerebral cortex, reduces motor activity, suppresses cerebral functions, including the respiratory center.
Decreases tone of smooth muscles of the gastrointestinal tract. It has anticonvulsant, sedative, hypnotic and antispasmodic action.
Caffeine increases reflex excitability of the spinal cord, excites respiratory and vasomotor centers, stimulates metabolic processes in organs and tissues including muscular tissue and CNS.
Papaverine hydrochloride is an antispasmodic and has hypotensive effect. It reduces tone and relaxes smooth muscles of internal organs and vessels.
Calcium gluconate replenishes the lack of calcium ions necessary for the process of transmission of nerve impulses.
Bromisoval has sedative and mild sleeping pills.

How to take, the dosage
The drug is taken orally after meals.
Pagluferal®-1 is administered in children aged 3-4 years 1 tablet. 2 times a day; children aged 5-6 years – 1.5 tablets. 2 times a day; children aged 7-9 years – 2 tablets. 2 times a day; children aged 10-14 years – 3 tablets. 2 times a day; children from 14 years of age and adults – 4 tablets. 2 times a day.
Pagluferal®-2 is indicated for children aged 3-4 years old – 1 tablet. 2 times a day; children aged 5-6 years – 1.5 tablets. 2 times a day; children aged 7-9 years – 2 tablets. 2 times a day; children aged 10-14 years – 2.5 tablets. 2 times a day; children from 14 years of age and adults – 4 tablets. 2 times/day.
Pagluferal®-3 is indicated for children aged 7-9 years – 1 tablet. 2 times a day; children aged 10-14 years – 1.5 tablets. 2 times a day; children from 14 years of age and adults – 2 tablets. 2 times per day.List B. The drug should be kept in dry place, protected from light and out of reach of children at temperatures not exceeding 25 ° C.
Interaction
Phenytoin and valproates increase the serum content of phenobarbital.
The effect of phenobarbital is reduced when concomitant administration with reserpine, increased when combined with amitriptyline, nialamide, diazepam, chlordiazepoxide.
Decreases the effectiveness of oral contraceptives and salicylates.
Reduces blood levels of indirect anticoagulants, GCS, griseofulvin, doxycycline, estrogen and other drugs that are metabolized in the liver by oxidation (accelerates their metabolism).
It enhances the effect of ethanol, neuroleptics, narcotic analgesics, muscle relaxants, sedatives and hypnotics.
Acetazolamide, by alkalizing the urine, reduces reabsorption of phenobarbital in the kidneys and weakens its effect.
Decreases the antibacterial activity of antibiotics and sulfonamides and the antifungal effect of griseofulvin.
Special Instructions
In patients with mild hypercalciuria, decreased glomerular filtration or with nephrolithiasis in anamnesis the drug administration should be carried out under monitoring of urinary calcium content due to the presence of calcium gluconate in the drug.
The use of alcohol during therapy is not recommended.
Impact on driving and operating machinery
When taking this medicine, patients should refrain from performing any work requiring quick psychomotor reactions (including driving a vehicle).
Contraindications
– significant hepatic and/or renal impairment;
– drug dependence (including a history of history);
– hyperkinesias;
– myasthenia;
– severe anemia;
– porphyria;
– diabetes mellitus;
– adrenal hypofunction;
– hyperthyroidism;
p> – depression;
– bronchoobstructive disorders;
– active alcoholism;
– pregnancy;
– lactation period;
– hypersensitivity to any of the ingredients of the drug.
This drug form is not used in children under 3 years of age.
Perhaps caution should be exercised when using the drug in frail patients (high risk of paradoxical agitation, depression, and confusion even when prescribed in usual doses).
Side effects
Nervous system disorders: dizziness, general weakness, ataxia, dysarthria, nystagmus, paradoxical reaction (especially in elderly and weakened patients – agitation), hallucinations, sleep disorders.
Digestive system disorders: nausea, vomiting, constipation or diarrhea, increased liver transaminase activity.
Hematopoietic system disorders: agranulocytosis, megaloblastic anemia, thrombocytopenia.
Cardiovascular system: decrease or increase in BP, tachycardia, arrhythmia (including extrasystole), AV-blockade.
Others: allergic reactions, with prolonged use – drug dependence.
Overdose
Symptoms: increased severity of side effects.
Treatment: gastric lavage, administration of activated charcoal, symptomatic therapy.
