Nicotinic acid – a water-soluble vitamin B, replenishes the deficiency of vitamin PP (vitamin B3), is a specific antipellagic agent (vitamin PP avitaminosis).
It has a vasodilatory effect at the level of small vessels (including in the brain), improves microcirculation, has a weak anticoagulant effect, activates the fibrinolysis system. It has anti-atherogenic properties.
Pharmacokinetics
In parenteral administration it is rapidly distributed in the body tissues. It accumulates mainly in the liver, but also in fatty tissue and the kidneys. In the liver nicotinic acid is converted into an amine, which is incorporated into nicotinamide adenine dinucleotide (NAD), which is a prosthetic group of enzymes that carry water and carry out redox processes.
The main metabolites are H-methyl-2-pyridone-3-carboxamide and N-methyl-2-pyridone-5-carboxamide, which have no pharmacological activity. It can be synthesized in the gut by bacterial flora from ingested tryptophan (from 60 mg of tryptophan 1 mg of nicotinic acid is formed) with the participation of pyridoxine (vitamin B6) and riboflavin (vitamin B2).
The elimination half-life is 45 minutes. It is eliminated from the body by the kidneys in unchanged form and as metabolites, while taking high doses – mainly unchanged. Renal clearance depends on the level of nicotinic acid in plasma and may decrease when its concentration in plasma is high.
Indications
– RR avitaminosis (pellagra).
– Complex therapy: ischemic stroke; obliterating vascular diseases of the extremities (obliterating endarteritis, Raynaud’s disease); neuritis of the facial nerve.
– Hartnup disease (a hereditary disease accompanied by impaired absorption of certain amino acids, including tryptophan).

How to take, the dosage
The drug is used subcutaneously, intramuscularly, intravenously (slowly).
In case of pellagra (PP avitaminosis) parenterally administer 1 ml of 1% (10 mg) solution 1-2 times a day for 10-15 days.
In case of ischemic stroke 10 mg of solution is slowly administered intravenously.
In Hartnup disease: 40-200 mg per day.
For the other indications listed under “Indications for use,” 10 mg 1-2 times daily, for 10-15 days.
The maximum doses for adults: single dose 100 mg, daily 300 mg.
Interaction
Caution should be exercised when combining with hypotensive agents, anticoagulants and acetylsalicylic acid.
Nicotinic acid reduces toxicity of neomycin and prevents its induced reduction of cholesterol and high density lipoproteins.
When used together with sulfonylurea drugs may increase blood glucose levels.
Ingestion with lovastatin increases the risk of myopathy.
Special Instructions
In the course of treatment, liver function should be monitored regularly. With prolonged use of nicotinic acid in high doses, fatty liver infiltration may develop. To prevent hepatotoxicity it is necessary to include in the diet foods rich in methionine (cottage cheese), or to prescribe methionine or other lipotropic agents.
Impact on the ability to drive:
During treatment, it is recommended to refrain from driving and engaging in other potentially hazardous activities requiring increased concentration and rapid psychomotor reactions.
Contraindications
Hypersensitivity to the drug components, severe arterial hypertension, atherosclerosis, gout, hyperuricemia, hepatitis, decompensated diabetes, gastric and duodenal ulcer (acute stage), children (under 18 years).
With caution:
Hemorrhagia, glaucoma, hepatic insufficiency, arterial hypotension, hyperacid gastritis, gastric and duodenal ulcer (out of acute stage).
Overdose
Overdose is unlikely due to the low toxicity of the drug. Nicotinic acid in individuals with individual intolerance may cause redness of the face and upper half of the trunk, dizziness, feeling of hot flashes, urticaria, paresthesias (numbness, loss of sensitivity, tingling, crawling of goose bumps).
Treatment: symptomatic.
