Nicotinic acid is a specific antipellagic agent.
Has a pronounced short-term vasodilator, improves carbohydrate and nitrogen metabolism, has hypolipidemic activity, reduces total cholesterol, low-density lipoproteins, triglycerides, increases high-density lipoproteins, improves microcirculation.
In the body nicotinic acid is converted to nicotinamide, which binds to codohydrase I and codohydrase II coenzymes (NAD and NADPH), carrying hydrogen, is involved in metabolism of fats, proteins, amino acids, purines, tissue respiration, glycogenolysis, synthetic processes.
It compensates vitamin PP (vitamin B3) deficiency being a specific antipellagic agent.
It has vasodilator effect at the level of small vessels (including brain), improves microcirculation, has weak anticoagulant effect (increases blood fibrinolytic activity).
Indications
Cerebral circulation disorders, Raynaud’s disease, hepatitis, diabetic nephropathy, neuritis, hypovitaminosis, vascular lesions in diabetes – hypovitaminosis PP, avitaminosis PP (pellagra);
– spasms of limb vessels;
– as a part of combined therapy for ischemic stroke;
– Illnesses of limb vessels obliterating (obliterating endarteritis, Raynaud’s disease);
– Neuritis of facial nerve;
– Diabetes, including its complications (diabetic polyneuropathy, microangiopathy);
– Hartnup disease (hereditary disease accompanied with disorders of assimilation of several amino acids including tryptophan).
Composition
Active ingredients:
nicotinic acid – 10 mg.
Auxiliary substances:
sodium bicarbonate – 7 mg,
water for injection – up to 1 ml.

How to take, the dosage
In pellagra in adults parenterally (intravenously, intramuscularly or subcutaneously) 1 ml of 1% (10 mg) 2-3 times a day for 10-15 days.
In ischemic stroke 10 mg of solution is slowly injected intravenously.
For other indications – 10 mg 1-2 times a day for 10-15 days.
Higher doses for adults: single – 100 mg, daily – 300 mg.
Hartnup Disease 40-200 mg per day.
Interaction
When combined with sulfonylureas may increase blood glucose levels.
When administered with lovastatin increases the risk of myopathy.
Caution should be exercised when combining with hypotensive drugs, anticoagulants and ascorbic acid.
It reduces toxicity of neomycin and prevents its induced decrease in concentration of cholesterol and high-density lipoproteins.
Special Instructions
To prevent complications from the liver is recommended to include in the diet foods rich in methionine (cottage cheese), or use methionine, lipoic acid and other lipotropic agents.
Taking into account the injection route of the drug, side effects, there is no need for use during pregnancy.
Contraindications
Atherosclerosis, hypersensitivity to Nicotinic acid, gastric and duodenal ulcer (acute stage), gout, hyperuricemia, hepatitis, liver cirrhosis, decompensated diabetes.
With caution: hemorrhage, glaucoma, hyperacid gastritis, peptic ulcer (out of acute stage), arterial hypotension, pregnancy, lactation, children.
