Combination drug, the effect of which is determined by its constituent components
Caffeine has psychostimulant (stimulates the psychomotor centers of the brain), analeptic effect, enhances the effect of analgesics, eliminates sleepiness and fatigue, increases physical and mental performance.
Paracetamol has antipyretic and analgesic effect (due to inhibition of COX and inhibition of prostaglandin synthesis it affects the center of thermoregulation in the hypothalamus and the conduction of pain impulses in the CNS).
Pharmacokinetics
Paracetamol
Absorption is high, binding to plasma proteins – 15%. Time to reach maximum concentration (TC max ) – 20-30 min. It penetrates through the blood-brain barrier.
Less than 1% of the dose of paracetamol taken by a nursing mother passes into breast milk.
Therapeutically effective plasma concentrations of paracetamol are achieved when administered at a dose of 10-15 mg/kg.
Metabolized in liver: 80% reacts in conjugation with glucuronic acid and sulfates to form inactive metabolites: 17% undergoes hydroxylation to form active metabolites, which conjugate with glutathione and form inactive metabolites.
In glutathione deficiency these metabolites can block the enzyme systems of hepatocytes and cause their necrosis.
The elimination half-life (T 1/2 ) is 2-3 hours.
In elderly patients the drug clearance is decreased and T 1/2 is increased.
Extracted by the kidneys – 3% unchanged.
Caffeine
Tc mah – caffeine – 1h; T 1/2 – 3.5h; 65-80% of the drug is excreted mainly as 1-methylxanthine, 1-methylureic acid and acetylated derivatives of uracil a small amount is converted to theophylline and theobromine.

How to take, the dosage
I take Migrenol tablets orally, usually 1-2 tablets, if necessary 3-4 times a day. The interval between doses is at least 4 hours. The maximum single dose is 2 tablets, the maximum daily dose is 8 tablets.
The drug should not be used without prescription and medical supervision for more than 5 days if it is used as an analgesic and for more than 3 days as an antipyretic.
The indicated dose should not be exceeded. In case of overdose, it is necessary to consult a doctor immediately.
Interaction
When taken for a long time, the drug increases the effect of indirect anticoagulants (warfarin and other coumarins), which increases the risk of bleeding.
Hepatic microsomal oxidation inducers (barbiturates, phenytoin, ethanol, rifampicin, phenylbutazone, tricyclic antidepressants) increase the risk of hepatotoxic effects in small overdoses.
Microsomal oxidation inhibitors (cimetidine) reduce the risk of hepatotoxic effects.
The elimination time of chloramphenicol is 5 times longer under the influence of paracetamol.
Caffeine accelerates absorption of ergotamine.
The simultaneous use of paracetamol and alcoholic beverages increases the risk of hepatotoxic effects and acute pancreatitis.
Metoclopramide and domperidone increase and colestyramine decreases the rate of absorption of paracetamol.
The drug may decrease the effectiveness of uricosuric drugs.
It enhances the effect of monoamine oxidase inhibitors.
Special Instructions
Excessive consumption of caffeine-containing products (coffee, tea) during treatment may cause overdose symptoms. During treatment, avoid work requiring quick reaction and concentration, do not use alcohol and other CNS inhibitors.
Peripheral blood count and liver function monitoring are required during long-term treatment.
Impact on the ability to drive and operate machinery
The drug Migrenol may cause decreased speed of motor reactions. During treatment, avoid work requiring fast reactions and concentration.

Side effects
The drug is usually well tolerated in the recommended doses.
In prolonged use at high doses
Hepatotoxic effects, hemolytic anemia, aplastic anemia, methemoglobinemia, pancytopenia, nephrotoxicity.
Overdose
Symptoms: nausea, vomiting, stomach pain, pale skin, anorexia. Hepatonecrosis (severity of necrosis due to intoxication directly depends on the degree of overdose), the unfolded clinical picture of liver damage is manifested in 1-6 days.
Treatment: gastric lavage and adsorbents (activated charcoal). If an overdose is suspected, seek medical attention immediately.
