Meldonium is an analogue of gamma-butyrobetaine, inhibits gamma-butyrobe-tainhydroxinase, reduces carnitine synthesis and transport of long-chain fatty acids through cell membranes, prevents accumulation of activated forms of unoxidized fatty acids – derivatives of acylcarnitine and acylcoenzyme A in cells.

As a result of a decrease in the concentration of carnitine enhanced synthesis of gamma-butyrobetaine with vasodilatory properties.

Under ischemic conditions it restores the balance of oxygen delivery and consumption in the cells, prevents the violation of ATP transport, while simultaneously activating glycolysis, which proceeds without additional oxygen consumption.

The mechanism of action determines the diversity of its pharmacological effects: increase efficiency, reduction of symptoms of mental and physical stress, activation of tissue and humoral immunity, cardioprotective effect. In acute ischemic myocardial injury slows the formation of necrotic zone, shortens the rehabilitation period.

In heart failure increases myocardial contractility, increases exercise tolerance, reduces the frequency of angina attacks. In acute and chronic ischemic cerebral circulatory disorders it improves blood circulation in ischemic area and redistributes the blood in favor of ischemic area.

Effective in cases of vascular and dystrophic eye fundus pathology. It is also characterized by tonic effect on the central nervous system, elimination of functional disorders of somatic and vegetative nervous systems in patients with alcoholism during withdrawal.

Indications

As part of the combined therapy of coronary heart disease, chronic heart failure, cardialgia against myocardial dystrophy.

How to take, the dosage

Overly, before meals.

Stable angina pectoris, chronic heart failure – as part of complex therapy at 500-1000 mg a day, using the whole dose at once or dividing it into 2 times. The course of treatment is 4-6 weeks.

Cardialgia against myocardial dyshormonal dystrophy: 250 mg 2 times a day (in the morning and in the evening). The course of treatment is 12 days.

Brain blood supply disorders (stroke, cerebrovascular insufficiency) – 500-1000 mg per day, preferably in the morning. The course of treatment is 4-6 weeks. Withdrawal alcohol syndrome – 500 mg 4 times a day. The course of treatment is 7-10 days.

With decreased performance and chronic physical stress, in the postoperative period to accelerate the rehabilitation – 250 mg 4 times a day. The course of treatment is 10-14 days. If necessary the treatment is repeated in 2-3 weeks.

Sportsmen are prescribed 500-1000 mg 2 times a day before training. The course duration during the preparatory period is 14-21 days, during the competition period it is 10-14 days.
