Pharmacodynamics
The active ingredient of Likopid® tablets – glucosaminylmuramyldipeptide (GMDP) – is a synthetic analog of the structural fragment of the shell (peptidoglycan) of bacterial cells. GMDP is an activator of innate and acquired immunity; it enhances the body’s defense against viral, bacterial and fungal infections; it has an adjuvant effect in the development of immunological reactions.
Likopid® has low toxicity (LD50 exceeds the therapeutic dose by more than 49 000 times). In experiment at oral administration with doses 100 times higher than the therapeutic one the drug does not have toxic effect on the central nervous and cardiovascular systems and does not cause pathological changes of internal organs. Lycopid® has no embryotoxic and teratogenic effect and does not cause chromosomal and gene mutations. In experimental studies performed on animals the data about the antitumor activity of Lycopid® (GMDP) was received.
Pharmacokinetics
The bioavailability of the drug with oral administration is 7-13%. The degree of binding to blood albumin is weak. Time of reaching maximum concentration (tmax) is 1.5 hours after intake. Elimination half-life (t1/2) is 4.29 hours. It does not form active metabolites, it is excreted mainly through the kidneys unchanged.

Composition
Active ingredient: glucosaminylmuramyldipeptide (GMDP) – 10.0 mg. Supplementary substances: lactose monohydrate – 184.7 mg, sucrose (sugar) – 12.5 mg, potato starch – 40 mg, methylcellulose – 0.3 mg, calcium stearate – 2.5 mg.
How to take, the dosage
Lycopid® is taken orally on an empty stomach, 30 minutes before a meal.
In elderly patients it is recommended to start treatment with half doses (1/2 of the therapeutic dose), increasing to the required therapeutic dose if there are no side effects.
If you miss a dose, you can take the missed dose if it is less than 12 hours from the scheduled time; if it is more than 12 hours from the scheduled time, you should only take the next dose and not the missed dose.
Inflammatory diseases of the skin and soft tissues acute and chronic, severe, including purulent-septic postoperative complications: 10 mg once daily for 10 days;
Herpetic infection (relapsing course, severe forms): for 10 mg once daily for 6 days;
– in ophthalmoherpes: for 10 mg 2 times daily for 3 days. After a break of 3 days the course of treatment is repeated.
Infections transmitted sexually (papillomavirus infection, chronic trichomoniasis):
Psoriasis: 10-20 mg once a day for 10 days and then five doses every other day of 10-20 mg once a day.
In severe course of psoriasis and extensive lesions (including psoriatic arthritis): 10 mg 2 times daily for 20 days.
Pulmonary tuberculosis: 10 mg once daily for 10 days.
Precautionary measures for use:
Each tablet of Lycopid® 10 mg contains 0.001 x.u. (bread units) sucrose, which should be considered in patients with diabetes.
Each Lycopid® 10 mg tablet contains 0.184 grams of lactose, which should be considered
patients with hypolactasia (lactose intolerance, in which the body has decreased levels of lactase, an enzyme needed to digest lactose).
With caution:
Likopid® 10 mg is used with caution in the elderly, strictly under the supervision of a physician.
Interaction
Special Instructions
At the beginning of taking Likopid® 10 mg an exacerbation of symptoms of chronic and latent diseases related to the main pharmacological effects of the drug is possible.
In elderly patients Livopid® 10 mg is used with caution, under strict medical supervision. For elderly patients it is recommended to start the treatment with half dose (1/2 of the therapeutic dose) and in case of absence of side effects increase the dose up to the required therapeutic dose.
The decision of whether to prescribe Likopid® 10 mg tablets to patients with a combination of diagnoses “psoriasis” and “gout” should be made by physician when assessing the risk/benefit ratio due to the potential risk of gouty arthritis exacerbation and joint edema. If doctor decides to prescribe Lycopid® 10 mg tablets when a patient has a combined diagnosis of psoriasis and gout, treatment should be started with a low dose, in the absence of side effects, increasing the dose to therapeutic.

Contraindications
Hypersensitivity to glucosaminylmuramyldipeptide and other ingredients of the drug;
pregnancy and breastfeeding; children under 18 years of age;
Autoimmune thyroiditis in the acute phase;
conditions accompanied by fever (>38°C) at the time of taking the drug;
rarely occurring congenital metabolic disorders: alactasia, galactosemia, lactase deficiency, lactose intolerance, sucrose/isomaltase deficiency, fructose intolerance, glucose-galactose malabsorption;
The use in autoimmune diseases is not recommended due to lack of clinical data.
Side effects
Frequently (1-10%) – arthralgia (joint pain), myalgia (muscle pain); at the beginning of treatment a short-term increase in body temperature to subfebrile values (up to 37.9 ° C) may be noted, which is not an indication for discontinuation of the drug. Most often the above mentioned side effects are observed when taking Lycopid® tablets in high doses (20 mg).
Rarely (0.01-0.1%) Elevation of body temperature up to febrile values (>38,0 °С). If the body temperature is higher than 38.0 ° C, it is possible to take antipyretic drugs, which do not reduce the pharmacological effects of Lycopid® tablets.
very rarely (less than 0.01%) – diarrhea.
