Hydrocortisone is a natural glucocorticosteroid.
It has anti-inflammatory, anti-allergic effect. It reduces inflammatory cell infiltrates, decreases migration of leukocytes and lymphocytes in the inflamed area.
Stabilizes cellular and subcellular, including lysosomal membranes and membranes of mast cells.
Reduces the binding of immunoglobulins to receptors on cell surfaces and inhibits the synthesis or release of cytokines (interleukins and interferon) from lymphocytes and macrophages. Reduces the release of arachidonic acid from phospholipids and the synthesis of its metabolites (prostaglandins, leukotrienes, thromboxane).
Decreases the exudative reaction, helps to reduce capillary permeability.
Decreases the severity of the early immunological response. It has a catabolic effect and inhibits the development of connective tissue and scarring.

Composition
The 100 g ointment contains:
the active substance:
hydrocortisone acetate – 0.5 g;
excipients:
methylparahydroxybenzoate (methylparaben),
Vaseline.
How to take, the dosage
Eye ointment: topically. 1-2 cm of the eye ointment is injected into the conjunctival sac 2-3 times a day.
The course of treatment is 1 – 2 weeks. The course of treatment can be prolonged upon the doctor’s recommendation.
Interaction
The effect is weakened by barbiturates, hypokalemia is increased by diuretics. Reduces the effect of hypoglycemic agents.
In prolonged use of the drug with possible resorption of the drug into the general bloodstream, hydrocortisone reduces the effectiveness of insulin, oral hypoglycemic drugs, hypotensive agents, anticoagulants, reduces the concentration of salicylates in the blood, reduces the concentration of praziquantel in the blood serum.
Add to the risk of side effects of hydrocortisone: Androgens, estrogens, oral contraceptives, anabolic steroids (hirsutism, acne); antipsychotics, carbutamide, azathioprine (cataracts); choline blockers, antihistamines, tricyclic antidepressants, nitrates (glaucoma); diuretics (hypokalemia).
When used with cardiac glycosides the development of digitalis intoxication is possible. Simultaneous use with preparations of lead and silver leads to mutual inactivation.
Special Instructions
Wearing contact lenses is not recommended during treatment. When using eye drops the time interval between their instillation and application of the ointment – at least 15 minutes. When using for more than 10 days and if there is a history of open- or closed-angle glaucoma it is necessary to monitor the intraocular pressure. Shelf life after opening the tube – 1 month.
Effect on the ability to drive vehicles and operate machinery.
If after the use of the drug the patient’s vision is temporarily reduced, until its restoration it is not recommended to drive vehicles and engage in activities that require clarity of vision.
Contraindications
Side effects
Allergic reactions, burning, injected sclerae, momentary blurring of vision.
When prolonged use for more than 10 days may be observed:
– increase in intraocular pressure with possible subsequent development of steroid glaucoma with optic nerve damage and visual field disruption (therefore, when using drugs containing glucocorticosteroids for more than 10 days, intraocular pressure should be measured regularly);
– formation of posterior subcapsular cataract;
– delayed wound healing (in diseases that cause corneal thinning its perforation is possible). Secondary bacterial infection may occur due to suppression of the protective reaction of the patient’s body. In acute purulent diseases of the eye, glucocorticosteroids may mask or intensify an existing infectious process.
Fungal corneal lesions tend to occur particularly often with long-term use of glucocorticosteroids. The appearance of non-healing ulcers on the cornea after long-term treatment with glucocorticosteroids may indicate the development of a fungal infestation.
Pregnancy use
There have been no studies on the safety and efficacy of hydrocortisone in pregnant and lactating women.
There are no definitive data on the penetration into the breast milk of glucocorticosteroids when used topically.
However, the risk cannot be completely excluded.
The use of hydrocortisone in pregnant and lactating women is allowed only when prescribed by the treating physician when the expected therapeutic benefit to the mother exceeds the potential risk to the fetus and baby.
