Pharmacodynamics
Glucocorticosteroid has anti-inflammatory and anti-allergic effect. It reduces inflammatory cellular infiltrates, decreases migration of leukocytes and lymphocytes into the inflamed area. Stabilizes cellular and subcellular, including lysosomal membranes and membranes of mast cells. Reduces immunoglobulin binding to cell surface receptors and inhibits synthesis or release of cytokines (interleukins and interferon) from lymphocytes and macrophages. Reduces the exudative reaction, reduces capillary permeability. It reduces the severity of the early immunological response.

It has antimetabolic effect and inhibits the development of connective tissue and scarring.

Pharmacokinetics
Hydrocortisone poorly penetrates through cornea into the intraocular fluid; penetrates into epidermis and mucous membrane epithelium, can be slightly absorbed into systemic blood flow and has systemic effect.
Metabolized directly in epidermis and mucous membrane epithelium, and after absorption a small amount of it enters the general bloodstream and is metabolized in the liver. In the blood, 80% is bound to transcortin and 10% to albumin. Hydrocortisone metabolites are excreted by kidneys and intestines.

Special Instructions

Wearing contact lenses is not recommended during treatment. When concomitant use of eye drops the time interval between their instillation and application of the ointment should be at least 15 min.

In case of use for more than 10 days and if there is a history of open-closed-angle glaucoma, it is necessary to monitor the intraocular pressure.

If patients temporarily lose visual acuity after using the drug, they should not drive a vehicle or engage in activities that require visual acuity until it is restored.

Contraindications

Hypersensitivity, purulent, viral, tuberculosis, fungal eye disease, glaucoma, trachoma, vaccination period, disorders of the integrity of the membranes of the eye, childhood (under 18 years).

Side effects

Overdose

Overdose is extremely rare. Symptoms: local side effects may occur. When the drug is discontinued, the overdose phenomena go away on their own.

Pregnancy use

There have been no studies on the safety and efficacy of Hydrocortisone in pregnancy and lactation. There are no definitive data on the penetration into the breast milk of glucocorticosteroids when used topically, but the risk cannot be completely excluded. Hydrocortisone may be used during pregnancy and lactation only when the expected therapeutic effect exceeds the potential risk to the fetus or child. The duration of use in these cases should not exceed 7-10 days.
