Pharmacological action
Hepabene is a combination drug, which includes substances of herbal origin – extract of chokecherry and milk thistle extract.
Pharmacodynamics
The extract of melanax contains fumarin alkaloid with choleretic action. By normalizing the motor-evacuation function of the biliary tract (biliary) and tone of the sphincter of Oddi, fumarin facilitates the flow of bile into the duodenum.
The milk thistle fruit extract contains silymarin – a mixture of flavolignans – which has a hepatoprotective effect in acute and chronic intoxications due to the ability to bind free oxygen radicals, hepatotoxic substances and the presence of moderate antioxidant and membrane stabilizing properties.
Reproductive toxicity, genotoxicity and carcinogenicity studies of the active ingredients of the drug have not been conducted.
Studies of the general toxicity of the extract of melilot and the extract of milk thistle fruits under repeated use in the experiment and tolerability under clinical conditions showed no appreciable toxic effects and no danger to humans.
Pharmacokinetics
. After oral administration, the flavolignans constituent of silymarin undergo intestinal-hepatic recirculation, being excreted mainly with the bile and, to a lesser extent, with the kidneys.
Indications
A herbal medicine to stimulate bile secretion and relieve symptoms of functional dyspepsia, such as: a feeling of bloating, flatulence.
The use of the drug Gepabene is based on many years of experience in its use for this indication.

Impact on the ability to drive vehicles and machinery
There is no data on the effect of the drug on the ability to drive vehicles and engage in other potentially hazardous activities that require increased concentration and speed of psychomotor reactions.

Composition
1 capsule contains:
active components: fumarica medicinal herb dry extract (corresponding to 4.13 mg of the sum of fumaric alkaloids in terms of protopine) 275.10 mg; milk thistle fruit dry extract (corresponding to 50 mg of silymarin, including at least 22 mg of silybin)* 83.33-100.00 mg;
excipients: microcrystalline cellulose (type 102)* 63.20-79.87 mg, corn starch 20.10 mg, talc 8.40 mg, macrogol-6000 4.50 mg, copovidone 4.50 mg, colloidal silicon dioxide 2.10 mg, magnesium stearate 2.10 mg; shell: gelatin 79.94 mg, yellow iron oxide dye 0.67 mg, red iron oxide dye 0.15 mg, black iron oxide dye 0.04 mg, titanium dioxide 1.29 mg, purified water 13.92 mg.
* The amount of milk thistle fruit extract dry depends on its actual activity and should contain 50 mg of silymarin. Microcrystalline cellulose is added in the amount necessary to compensate and to obtain the contents of the capsule with a theoretical weight of 480.00 mg.
Pregnancy
Due to the lack of special studies, the use of the drug during pregnancy and breastfeeding is contraindicated.
Contraindications
– increased individual sensitivity to the components of the drug, as well as to other plants of the Aster family (Asteraceae);
– acute inflammatory diseases of the liver, cholelithiasis, acute and chronic diseases of the biliary tract;
– children under 18 years of age.
With caution
Prescribe with caution to patients with hormonal disorders (endometriosis, uterine fibroids, breast, ovarian and uterine carcinoma, prostate carcinoma) due to the possible estrogen-like effect of silymarin.
Side Effects
Allergic reactions are possible.
A laxative effect and increased diuresis may occur.
There are no data regarding the incidence of adverse events.

Interaction
Drug interaction studies with Gepabene have not been conducted.
When silymarin is used together with oral contraceptives and drugs used in hormone replacement therapy, the effect of the latter may be reduced.
Silymarin can enhance the effects of drugs such as diazepam, alprazolam, ketoconazole, lovastatin, atorvastatin, vinblastine due to its inhibitory effect on isoenzymes of the cytochrome P450 system.

Overdose
To date, no cases of overdose have been registered.
In case of accidental ingestion of a high dose, it is necessary to induce vomiting, rinse the stomach, take activated charcoal, and, if necessary, carry out symptomatic treatment.
