Enam® refers to medicines affecting the renin-angiotensin-aldosterone system (RAAS),
– ACE inhibitors. Enam® is used for treatment of essential hypertension (primary arterial hypertension (AH))
Indications
Essential hypertension of any severity.
Renovascular hypertension.
Heart failure of any severity.
In patients with clinical manifestations of HF, Enam® is also indicated for:
increasing patient survival;
slowing the progression of heart failure;
reducing the frequency of hospitalizations for HF.
Prevention of the development of clinically significant heart failure.
In patients without clinical symptoms of heart failure with left ventricular dysfunction, Enam® is indicated for:
slowing down the development of clinical manifestations of heart failure;
reducing the frequency of hospitalizations for HF.
Prevention of coronary ischemia in patients with left ventricular dysfunction.
Enam® is indicated for:
reducing the incidence of myocardial infarction;
reducing the frequency of hospitalizations for unstable angina.

Pregnancy
The use of Enam® during pregnancy is not recommended.
Contraindications
Hypersensitivity to any of the components of the drug.
Side Effects
In general, Enam® is well tolerated.
In clinical studies, the total incidence of adverse events when using enalapril did not exceed that when taking placebo. In most cases, adverse events were mild, transient and did not require discontinuation of therapy.
When using enalapril, the following adverse events were observed (very common: ≥ 10%, common: ≥ 1% and < 10%, uncommon: ≥ 0.1% and < 1%, rare ≥ 0.01% and < 0.1%, very rare: < 0.01%, frequency unknown: frequency cannot be estimated based on the available data):
Interaction
When Enam is prescribed simultaneously with NSAIDs, the hypotensive effect of enalapril may be reduced;
with potassium-sparing diuretics (spironolactone, triamterene, amiloride) – hyperkalemia may develop;
with lithium salts – slowing down the excretion of lithium (monitoring the concentration of lithium in the blood plasma is indicated).
Ethanol enhances the hypotensive effect of the drug. When used simultaneously with antipyretic and analgesic drugs, the effectiveness of Enam may be reduced. Enalapril weakens the effect of drugs containing theophylline.
Cimetidine prolongs the effect of enalapril.
Concomitant use with diuretics, beta-blockers, methyldopa, nitrates, calcium channel blockers, hydralazine, prazosin enhances the hypotensive effect of enalapril.
The use of Enam together with anesthetic agents that have an antihypertensive effect can cause arterial hypotension.
Overdose
Symptoms: arterial hypotension.
Treatment: the patient should be laid down, legs raised. In mild cases of overdose, the patient is prescribed a saline solution orally.
In more serious cases, measures aimed at stabilizing blood pressure are carried out in a hospital setting; saline solution or plasma substitutes are administered intravenously.
Hemodialysis may be used.
