Cyanocobalamin has hematopoietic and stimulates cellular metabolism.
Indications
Dermatitis, Hepatitis, Muscle pain, Crohn’s disease, Pain, Neuritis, Hypovitaminosis, Myalgia (muscle pain), Radiculitis, Alcoholism, Bleeding, Liver cirrhosis, Psoriasis, trigeminal neuralgia, Anemia, Back pain, Neuralgia
Chronic anemia occurring with cyanocobalamin deficiency (Addison-Birmer disease, alimentary macrocytic anemia), as well as in the complex treatment of iron deficiency, posthemorrhagic and aplastic anemia, anemia caused by toxic substances and/or drugs.
Diseases of the nervous system: polyneuritis, radiculitis, causalgia and neuralgia (including trigeminal neuritis), amyotrophic lateral sclerosis, peripheral nerve injuries, funicular myelosis, cerebral palsy, Down’s disease.
Skin diseases (psoriasis, photodermatosis, dermatitis herpetiformis, atopic dermatitis).
Cronic hepatitis, cirrhosis of the liver, liver failure.
Acute radiation sickness.
With a prophylactic purpose – when prescribing biguanides, para-aminosalicylic acid, ascorbic acid in high doses, gastric and intestinal pathologies with impaired absorption of cyanocobalamin (resection of part of the stomach, small intestine, Crohn’s disease, celiac disease, malabsorption syndrome, sprue).
How to take, the dosage
Cyanocobalamin is administered subcutaneously, intramuscularly, intravenously, and in funicular myelosis, amyotrophic lateral sclerosis – intralumbly.
Adults.
Subcutaneously in Addison-Birmer anemia – 100-200 mcg/d every other day; in funicular myelosis, macrocytic anemia with nervous system disorders – 400-500 mcg/d in the first week – daily, then with intervals between injections up to 5-7 days (folic acid is prescribed simultaneously); During remission, the maintenance dose is 100 mcg/day 2 times a month, and in the presence of neurological phenomena – 200-400 mcg 2-4 times a month.
In acute post-hemorrhagic and iron deficiency anemia – 30-100 microgram 2-3 times a week, in aplastic anemia – 100 micrograms until the occurrence of clinical and hematological improvement.
In case of hepatitis and cirrhosis of the liver – 30-60 mcg/day or 100 mcg every other day for 25-40 days.
In acute radiation sickness, diabetic neuropathy – 60-100 mcg daily for 20-30 days.
In funicular myelosis, amyotrophic lateral sclerosis, multiple sclerosis – intralumbarly 15-30 mcg, and with each subsequent injection the dose is increased (50 mcg, 100 mcg, 150 mcg, 200 mcg). Intralumbar injections are given every 3 days, for a total of 8-10 injections per course. During remission in the absence of manifestations of funicular myelosis for maintenance therapy we prescribe 100 micrograms twice a month, if there are neurological symptoms – 200-400 micrograms 2-4 times during a month.
To eliminate cyanocobalamin deficiency, administer intramuscularly or intravenously: for treatment, 1 mg daily for 1-2 weeks, a maintenance dose of 1-2 mg once a week to once a month; for prevention, 1 mg once a month. The duration of treatment is set individually.
Children.
The drug is administered subcutaneously in a dose of 1 mcg/kg, the maximum daily dose is 100 mcg. In case of alimentary anemia and anemia in prematurely born children – 30 micrograms daily by subcutaneous dose for 15 days, in case of aplastic anemia – 100 micrograms until clinico-hematological improvement.
In dystrophic conditions after diseases, Down’s disease and infantile cerebral palsy – subcutaneously 15-30 mcg every other day.
The doses and mode of administration depend on the pathology and range from 30 mcg to 100 mcg per day. The 500 mcg/ml dosage form is not used in children under 3 years of age.
Interaction
Pharmaceutically incompatible with ascorbic acid, heavy metal salts (inactivation of cyanocobalamin), thiamine bromide, pyridoxine, riboflavin (because the cobalt ion contained in the cyanocobalamin molecule destroys other vitamins).
Special Instructions
Vitamin B12 deficiency should be confirmed diagnostically before prescribing the drug, as it may mask folic acid deficiency.
Contraindications
Hypersensitivity to the drug components, thromboembolism, erythraemia, erythrocytosis, pregnancy (there are separate indications of a possible teratogenic effect of B vitamins in high doses), lactation, children under 3 years for this dosage form. Leber’s disease, nicotine amblyopia (increases the risk of neurodegenerative damage of the optic nerve).
Stress Angina FK III, benign and malignant neoplasms (except in cases accompanied by megaloblastic anemia and vitamin B12 deficiency), susceptibility to blood clots.
