Pharmacological action.

Pharmacodynamics.

The analeptic agent of mixed action. Mechanism of action consists of central and peripheral effects on the nervous system. The central mechanism is related to direct effect on vasomotor center of medulla oblongata, resulting in its excitation and mediated increase in blood pressure (especially if this center is initially inhibited). Peripheral effect is caused by excitation of carotid sinus chemoreceptors, which leads to increased frequency and depth of respiratory movements. There is no direct effect on the cardiovascular system.

Pharmacokinetics.

The absorption is high and does not depend on the route of administration. It is rapidly metabolized in the liver to form inactive metabolites, which are eliminated mainly by the kidneys.

Indications

Collapse, asphyxia (including newborns), shock during surgery and in the postoperative period, respiratory and circulatory depression in infectious diseases.

Composition

1 ml of the drug contains:

The active ingredient is nikethamide 250.0 mg;

the excipient is water for injection up to 1.0 ml.

How to take, the dosage

The drug is administered intravenously, intramuscularly and subcutaneously.

Adults and children over 14 years of age are given subcutaneously, intramuscularly and intravenously at a dose of 1-2 ml 1-3 times a day.

Intravenous administration is carried out slowly.

In children, subcutaneously, depending on age, in a dose of 0.1-0.75 ml.

Since subcutaneous and intramuscular injections of Cordiamine are painful, in order to decrease pain, depending on the situation, 1 ml of procaine (Novocaine) 0.5% solution is injected into the place of intended injection. The highest doses for adults subcutaneously: single – 2 ml, daily – 6 ml

Interaction

Enhances the effects of psychostimulants, antidepressants.

Decreases the effects of narcotic analgesics, hypnotics, antipsychotic and antiepileptic drugs, anxiolytics.

Decreases the effectiveness of nikethamide: aminosalicylic acid, opiniazide, phenothiazine derivatives and drugs for general anesthesia. The seizure effects of the drug are enhanced by reserpine and aminazine.

The prescriptive effect of nikethamide is increased by monoamine oxidase inhibitors.

Synopsis

Transparent colorless or slightly yellowish solution with a specific odor.

Contraindications

Hypersensitivity to the ingredients of the drug;

Low seizure threshold, seizures (including history of seizures);

Hyperthermia in children;

Pregnancy, lactation.

Side effects

Digestive system disorders: vomiting.

Nervous system disorders: anxiety, muscle contractions (starting with the circular muscles of the mouth).

Skin side: itching, facial hyperemia.

Cardiovascular system: arrhythmia.

Others: pain in the injection site, allergic reactions.

Overdose

Symptoms of overdose: increased severity of dose-dependent side effects, generalized tonic-clonic convulsions.

Management measures in case of overdose: treatment with symptomatic (including anticonvulsants).

Pregnancy use

There is no reliable data on the safety of using the drug during pregnancy and lactation.

It is not recommended to use this drug during pregnancy and lactation.
