The drug Buscopan has antispasmodic effect on smooth muscles of internal organs, reduces secretion of digestive glands.
As a quaternary ammonium derivative, it does not pass through the GEB, so it has no anticholinergic effect on the CNS.
Indications
spasm of smooth muscles of the gastrointestinal tract, stomach pain
gastritis with high acidity
peptic ulcer of the stomach and duodenum
Pharmacological effect
The drug Buscopan has an antispasmodic effect on the smooth muscles of internal organs and reduces the secretion of the digestive glands.
Being a quaternary ammonium derivative, it does not pass through the BBB, therefore it does not have an anticholinergic effect on the central nervous system.
Special instructions
The drug is prescribed with caution if there is a suspicion of intestinal obstruction (including pyloric stenosis), with urinary tract obstruction (including benign prostatic hyperplasia), with a tendency to tachyarrhythmia (including atrial fibrillation).
Use Buscopan with caution in persons whose activities require increased visual acuity.

Contraindications

Interaction
Buscopan may enhance the anticholinergic effect of tricyclic and tetracyclic antidepressants, antihistamines, antipsychotic drugs, quinidine, amantadine, disopyramide, anticholinergic drugs (for example, tiotropium bromide, ipratropium bromide, atropine-like compounds).
The simultaneous use of Buscopan® and dopamine antagonists (for example, metoclopramide) leads to a weakening of the effect of both drugs on the gastrointestinal tract.
Buscopan may enhance tachycardia caused by beta-agonists.
Overdose
Currently, cases of Buscopan overdose have not been described, so the following symptoms and recommendations are theoretical.
Symptoms: urinary retention, dry mouth, redness of the skin, tachycardia, decreased gastrointestinal motility, transient visual impairment.
Treatment: gastric lavage with activated carbon and then 15% magnesium sulfate solution. In the future, the use of cholinomimetics is indicated.
For glaucoma, pilocarpine (in the form of eye drops) is prescribed topically.
If necessary, cholinomimetics are prescribed for systemic use (for example, neostigmine is administered intramuscularly or intravenously at a dose of 0.5-2.5 mg); provide supportive and symptomatic therapy; in case of paralysis of the respiratory muscles – intubation and mechanical ventilation; for urinary retention – catheterization of the bladder.
