Anafranil is a tricyclic antidepressant, an inhibitor of norepinephrine and serotonin reuptake.
Anafranil acts on the depressive syndrome in general, including especially on such its typical manifestations as psychomotor retardation, depressed mood and anxiety. Clinical effect is usually observed after 2-3 weeks of treatment.
In addition, Anafranil has a specific (different from its antidepressant effect) effect in obsessive-compulsive disorders.
The action of Anafranil in chronic pain syndromes, whether or not due to somatic diseases, is probably due to the facilitation of nerve impulse transmission mediated by serotonin and noradrenaline.
Indications

Adult treatment of depressive conditions of different etiology, proceeding with different symptomatology: Endogenous, reactive, neurotic, organic, masked, involutionary forms of depression; depression in schizophrenic and psychopathic patients; depressive syndromes occurring in old age due to chronic pain syndrome or chronic somatic diseases; depressive mood disorders of reactive, neurotic or psychopathic nature; obsessive-compulsive syndromes; chronic pain syndrome; phobias and panic attacks; cataplexy, accompanying narcolepsy.

How to take, the dosage
The doses of the drug are chosen individually, taking into account the patient’s condition.
The goal of treatment is to achieve optimal effect with doses as low as possible and to increase doses cautiously, especially in elderly patients and adolescents who are generally more sensitive to Anafranil SR than patients in intermediate age groups.
The starting daily dose is 75 mg (25 mg 2-3 times a day)
Interaction
When co-administration of ethanol and drugs depressing CNS (including other antidepressants, barbiturates, benzodiazepines and general anesthetics) a significant increase in CNS depressant effect, respiratory depression and hypotensive effect are possible. Increases sensitivity to drinks containing ethanol.
Special Instructions
Hypokalemia should be eliminated before starting treatment with Anafranil.
Contraindications
– hypersensitivity to clomipramine and other components of the drug, cross-sensitivity to tricyclic antidepressants of dibenzazepine group;
– concurrent use of MAO inhibitors, as well as a period of less than 14 days before and after their use;
– concomitant use of selective MAO inhibitors of type A reversible action (such as moclobemide);
– recently suffered myocardial infarction;
– congenital QT interval prolongation syndrome;
– pregnancy and lactation.
Side effects
The side effects of Anafranil are usually mild and somewhat similar to the symptoms of depression. As a rule, these effects go away during treatment or after reducing and reaching the lowest active dosage. If the adverse reactions are serious and life-threatening, the medication should be stopped.
Overdose
The symptoms that develop in overdose of Anafranil are similar to those described in overdose of other tricyclic antidepressants.
Pregnancy use
The experience of using Anafranil in pregnancy is limited. Since there are anecdotal reports of a possible association between tricyclic antidepressants and fetal impairment, Anafranil should be avoided during pregnancy unless the expected effects of treatment to the mother clearly exceed the potential risk to the fetus.
