Amlodipine is a class II selective calcium channel blocker. Antihypertensive effect is due to direct relaxing effect on vascular smooth muscles.
It is assumed that antianginal action of amlodipine is related to its ability to dilate peripheral arterioles, it leads to decrease of PPS, reflex tachycardia does not occur. As a result, there is a decrease of myocardial oxygen demand and energy consumption by the heart muscle.
On the other hand, amlodipine appears to cause dilation of large caliber coronary arteries and coronary arterioles of both intact and ischemic areas of myocardium. This ensures the flow of oxygen to the myocardium during coronary artery spasms.
Indications
Arterial hypertension (including in combination therapy), stable angina pectoris, unstable angina pectoris (including Prinzmetal angina).
Active ingredient
Composition
1 tablet contains amlodipine (in the form of besylate) 5 mg;
excipients:
Potato starch,
lactose,
microcrystalline cellulose,
magnesium stearate,
calcium stearate.
How to take, the dosage
In adults, when taken orally, the initial dose is 5 mg once daily.
The dose can be increased if necessary.
The maximum dose is 10 mg/day if taken orally.
Interaction
In concomitant use of Amlodipine with other antihypertensive drugs may increase the hypotensive effect; with agents for inhalation anesthesia – possible increase in hypotensive effect; with lithium drugs – possible manifestations of neurotoxicity, including nausea, vomiting, diarrhea, ataxia, tremor and/or tinnitus; with sympathomimetics – possible decrease of hypotensive effect.
Indomethacin and other NSAIDs may decrease the hypotensive effect of amlodipine by inhibiting the synthesis of prostaglandins in the kidneys and/or causing fluid retention.
Special Instructions
With caution is used in patients with liver dysfunction, aortic stenosis, chronic heart failure.
Patients with impaired renal function and elderly patients do not require dose reduction. Amlodipine may be used to treat patients with dilated (non-ischemic) cardiomyopathy accompanied by a severe form of chronic heart failure.
There are no clinical data on the use of amlodipine in children.
Contraindications
Developed arterial hypotension. Hypersensitivity to dihydropyridine derivatives.
Side effects
Cardiovascular system disorders: peripheral edema, palpitation, skin hyperemia are possible; when used in high doses – arterial hypotension, arrhythmias, shortness of breath.
The digestive system: nausea, abdominal pain; rarely – gum hyperplasia.
CNS disorders: headache, fatigue, drowsiness, dizziness are possible.
Allergic reactions: skin rash, itching.

Overdose
Symptoms: marked BP decrease, excessive peripheral vasodilation, tachycardia.
Treatment: gastric lavage, administration of activated charcoal, maintenance of cardiovascular function, control of heart and lung function parameters, elevation of extremities, control of circulating blood volume and diuresis. To restore vascular tone – the use of vasoconstrictors (if there are no contraindications for their use); to eliminate the effects of calcium channel blockade – intravenous injection of calcium gluconate. Hemodialysis is ineffective.
