Zyprexa is an antipsychotic.
As a ligand of serotonin (2A/2C, 3, 6), dopamine (D1-D5), m-choline (m1-m5), histamine H1– and alpha1-adrenoreceptors, and selectively affects the limbic system.
Pharmacokinetics
It is well absorbed, Cmax in plasma in 5-8 h. Metabolized in the liver (by conjugation and oxidative pathways) to inactive metabolites. The main metabolite – 10-N-glucuronide – does not pass through the BBB. Mean T1/2 varies (depending on age and sex) between 21-54 h.
Indications
The acute form of schizophrenia and other psychoses, accompanied by pronounced positive (delirium, hallucinations, thought disorders, hostility, suspiciousness) and/or negative (blunted affect, emotional and social detachment, poverty of speech activity) symptoms.
Secondary affective symptoms on the background of schizophrenia and related disorders.

How to take, the dosage
Ingestion, regardless of meals.
The initial dose is from 10 mg/day, once.
Thereafter, the daily dose may vary from 5 to 20 mg/day, depending on the clinical condition.
Interaction
It reduces the effects of levodopa and dopamine agonists.
Active charcoal reduces bioavailability by 50-60% when taken orally.
Smoking and carbamazepine accelerate metabolism.
There are no obvious interactions with antacids, cimetidine, imipramine, warfarin, diazepam, lithium, theophylline, biperiden.
Contraindications
Hypersensitivity
Side effects
Frequent – somnolence, weight gain;
rare – dizziness, dry mouth, constipation, peripheral edema, orthostatic hypotension, akathisia, dystonia, eosinophilia (asymptomatic), slight and temporary increase in prolactin levels (gynecomastia, lactorrhea), liver transaminase activity.
Overdose
Symptoms: nausea, confused speech, drowsiness.
Mydriasis, respiratory depression, hypotension, extrapyramidal disorders have been reported in animal experiments.
Treatment: no specific antidote.
The treatment is symptomatic, with mandatory use of activated charcoal, ensuring good ventilation and oxygenation;
In case of collapse, intravenous norepinephrine (but not adrenomimetics with beta-stimulating activity – adrenaline, dopamine, etc.).).
Pregnancy use
Zyprexa is contraindicated in pregnancy, breastfeeding, and children under 18 years of age.
