Truxal is a neuroleptic, a derivative of thioxanthene. It has antipsychotic, pronounced sedative and moderate antidepressant effect.
Pharmacodynamics
The antipsychotic action of Truxal is associated with its blocking effect on dopamine receptors. Blockade of these receptors is also associated with antiemetic and analgesic properties of the drug. Truxal is able to block 5-NT< sub>2 – receptors, α< sub>1 – adrenoreceptors, and H< sub>1 – histamine receptors, which determines its adrenoblocking, hypotensive and antihistamine properties.
Pharmacokinetics
The bioavailability of chlorprotixen with oral administration is about 12%. Chlorprotixen is rapidly absorbed from the intestine, maximum serum concentration is reached after 2 hours. The elimination half-life is about 16 hours. Chlorprotixene penetrates the placental barrier and is excreted in small amounts with breast milk. Metabolites have no neuroleptic activity and are excreted with feces and urine.

How to take, the dosage
Psychosis, including schizophrenia and manic states. Treatment begins with 50 to 100 mg/day, gradually increasing the dose until optimal effect is achieved, usually to 300 mg/day. In isolated cases, the dose may be increased to 1200 mg/day. The maintenance dose is usually 100 to 200 mg/day.
The daily dose of Truxal is usually divided into 2 to 3 doses; given the pronounced sedative effect of Truxal, it is recommended that a smaller portion of the daily dose be prescribed during the day and a larger portion in the evening.
Hangover withdrawal syndrome in alcoholism and drug addiction.
The daily dose, divided into 2 – 3 doses, is 500 mg. The course of treatment usually lasts 7 days. After withdrawal symptoms disappear, the dose is gradually reduced. The maintenance dose of 15 to 45 mg/day helps to stabilize the condition, reducing the risk of another binge.
In elderly patients with hyperactivity, irritability, agitation, confusion, 15 to 90 mg/day is prescribed. The daily dose is usually divided into 3 doses.
In children for correction of behavioral disorders, Truxal is prescribed at the rate of 0.5 to 2 mg per 1 kg of weight.
Depressive states, neuroses, psychosomatic disorders. Truxal may be used for depression, especially combined with anxiety and tension, as an adjunct to antidepressant therapy or alone. Truxal may be prescribed for neurosis and psychosomatic disorders accompanied by anxiety and depressive disorders, up to 90 mg/day. The daily dose is usually divided into 2-3 doses. As taking Truxal is not addictive or addictive, it can be used for a long time.
Insomnia. Take 15 – 30 mg in the evening 1 hour before sleep.
Pain. The ability of Truxal to potentiate the effects of analgesics can be used in the treatment of patients with pain. In these cases, Truxal is prescribed together with analgesics in doses from 15 to 300 mg.
Interaction
The CNS depressant effect of chlorprotixene may be enhanced with concomitant use of ethanol and ethanol-containing drugs, anesthetics, opioid analgesics, sedatives, hypnotics, neuroleptics.
The anticholinergic effect of chlorprotixen is increased with simultaneous use of anticholinergic, antihistamine and antiparkinsonian agents.
The drug increases the effect of hypotensive drugs.
Simultaneous use of chlorprotixen and adrenaline may lead to arterial hypotension and tachycardia.
The use of chlorprotixen leads to a decrease in the threshold of seizure activity, which requires additional adjustment of the dose of antiepileptic drugs in patients with epilepsy.
The ability of chlorprotixene to block dopamine receptors reduces the effectiveness of levodopa.
Extrapyramid disorders may occur with concomitant use of phenothiazines, metoclopramide, haloperidol, reserpine.
Special Instructions
Truxal should be prescribed with caution in patients with epilepsy, parkinsonism, marked atherosclerosis of cerebral vessels, tendency to collapse, marked cardiovascular and respiratory failure, marked liver and renal function disorders, diabetes, prostatic hypertrophy.
The use of Truxal may lead to false positive results in urine immunobiological test for pregnancy, false increase in bilirubin levels in blood, changes in the QT interval on electrocardiogram.
During treatment with Truxal, it is recommended to refrain from drinking alcohol and avoid excessive insolation.
Truxal administration has a negative effect on activities requiring high speed of mental and physical reactions (e.g., driving vehicles, operating machinery, working at heights, etc.).
Contraindications
Side effects
Sleepiness, tachycardia, dry mouth, increased sweating, or accommodation difficulties. These side effects, which usually occur at the beginning of therapy, often disappear as therapy continues. Orthostatic hypotension may occur, especially when using Truxal in high doses.
Dizziness, dysmenorrhea, skin rashes, constipation are rare. Extrapyramidal symptoms are particularly rare.
In isolated cases of decreased seizure threshold, transient benign leukopenia and hemolytic anemia have been described. With long-term use, especially with high doses the following may be observed: cholestatic jaundice, galactorrhea, gynecomastia, decreased potency and libido, increased appetite, increased body weight.
Overdose
Symptoms. Drowsiness, hypo- or hyperthermia, extrapyramidal symptoms, seizures, shock, coma.
Treatment. Symptomatic and supportive. Gastric lavage should be performed as soon as possible, the use of a sorbent is recommended.
Measures should be taken to maintain respiratory and cardiovascular system activity. Adrenaline should not be used as it may cause a subsequent drop in blood pressure.
Convulsions can be controlled with diazepam and extrapyramidal symptoms with biperiden.
