How to take, the dosage

Intentionally, 1/4 tablet (250 mg/25 mg) 2-3 times/day, then the dose is increased by 1/4 tablet every 2-3 days until optimal effect is achieved. Usually the optimal effect is noted at a dose of 1-2 tablets (250 mg/25 mg) per day.

The maximum daily dose is 1.5 g of levodopa and 150 mg of carbidopa (6 tablets 250 mg/25 mg).

Interaction

In concomitant use of levodopa with beta-adrenergic stimulants, ditiline and drugs for inhalational anesthesia, there may be an increased risk of cardiac rhythm disturbances; with tricyclic antidepressants – reduced bioavailability of levodopa; with diazepam, clozapine, phenytoin, clonidine, m-cholinoblockers, antipsychotic drugs (neuroleptics) – butyrophenone derivatives, diphenyl butylpiperidine, thioxanthene, phenothiazine, and also with pyridoxine, papaverine, reserpine a decrease of antiparkinsonian action is possible; with lithium preparations – increased risk of dyskinesia and hallucinations; with methyldopa – exacerbation of side effects.

Simultaneous use of levodopa with MAO inhibitors (except for MAO type B inhibitors) may cause circulatory disorders (MAO inhibitors should be discontinued 2 weeks beforehand). This is due to accumulation of dopamine and norepinephrine under levodopa influence, inactivation of which is inhibited by MAO inhibitors, and high possibility of agitation, increase in blood pressure, tachycardia, facial flushing and dizziness.

The risk of significant BP decrease increases in patients receiving levodopa.

Iron salts may decrease the bioavailability of levodopa and carbidopa; the clinical significance of this interaction is unknown.

Although metoclopramide increases the bioavailability of levodopa by accelerating gastric emptying, it may still adversely affect disease control, due to its antagonism with dopamine receptors.
