Indications
bacterial skin infections caused by sensitive microorganisms, incl. infected burns (superficial and delimited deep)
bedsores
trophic ulcers
wounds (II phase of the wound process: absence of pus, necrotic tissue – for liniment devoid of osmotic activity)
boils
cracked nipples in nursing women.
Pharmacological effect

Chloramphenicol is a bacteriostatic antibiotic that disrupts the process of protein synthesis in the microbial cell (being highly lipophilic, it penetrates the bacterial cell membrane and reversibly binds to the

Active against most strains of gram-positive and gram-negative microorganisms resistant to penicillin, tetracyclines, and sulfonamides.

Pharmacokinetics: the extent of absorption into the systemic circulation after application of the drug to the skin, wounds or mucous membranes is unknown.
Special instructions
During treatment, systematic monitoring of peripheral blood patterns is necessary.
No adverse effects on the fetus have been identified during pregnancy or breastfeeding.
When applied to large surfaces with simultaneous intake of ethanol, the development of disulfiram-like reactions (skin hyperemia, tachycardia, nausea, vomiting, reflex cough, convulsions) is possible.

Composition
Active ingredient:
Chloramphenicol [D,L] (syntomycin) – 100 mg;
Excipients:
Castor oil – 200.0 mg,
Emulsifier No. 1 – 30.0-50.0 mg,
Sorbic acid – 2.0 mg,
Ethanol – 8.1 mg in terms of 96% alcohol,
Carmellose sodium – 18.4 mg in terms of degree of polymerization 450 – 500, water – up to 1 g.
Contraindications
Individual intolerance, suppression of bone marrow hematopoiesis, acute intermittent porphyria, glucose-6-phosphate dehydrogenase deficiency, liver and/or kidney failure, skin diseases (fungal infections, psoriasis, eczema), pregnancy, lactation, neonatal period (up to 4 weeks).
Due to the lack of osmotic activity (base: oil, emulsifier), it is not recommended for use in the first phase of the wound process (profuse suppuration, severe tissue swelling, pain and the presence of necrotic tissue).
With caution
Early childhood, in patients who have previously received treatment with cytotoxic drugs or radiation therapy.
Side Effects
Allergic reactions are possible: skin rash, angioedema.
From the hematopoietic organs:
rarely – reticulocytopenia, leukopenia, granulocytopenia, thrombocytopenia, erythropenia, aplastic anemia, agranulocytosis.
Interaction
Barbiturates accelerate biotransformation, reduce the concentration and duration of the effect.
Reduces the antibacterial effect of penicillins and cephalosporins.
When used simultaneously with erythromycin, clindamycin, lincomycin, a mutual weakening of the effect is observed.
With simultaneous consumption of alcohol, disulfiram-like reactions are possible.
