Supirocin-B, ointment 15 g

A combination drug for external use, the effect of which is due to its constituent components.

Mupirocin is an antibiotic produced by the microorganism Pseudomonas fluorescens. Mupirocin inhibits bacterial cell protein synthesis through reversible binding to isoleucyl-transport RNA synthetase, which prevents isoleucine incorporation into the building protein chains.

When used in minimal inhibitory concentrations, mupirocin has bacteriostatic and in higher concentrations it has bactericidal action.Due to its specific mechanism of action and unique chemical structure, there is no cross-resistance with other antibiotics. With proper use of mupirocin, the risk of resistant strains of microorganisms is low.

Betamethasone is a glucocorticosteroid for external use. It has a local anti-inflammatory, anti-allergic, anti-edema and anti-proliferative effect. It has stronger vasoconstrictor action than other fluorine derivatives of glucocorticosteroids; systemic effects are expressed insignificantly due to limited absorption through the skin.

In skin exposure there is prevention of marginal accumulation of neutrophils, which leads to reduction of exudation, cytokine production, inhibition of macrophage migration, leading ultimately to reduction of infiltration and granulation processes.

Indications

The treatment of atopic dermatitis, diffuse neurodermitis, contact dermatitis (simple and allergic), exfoliative dermatitis, urticaria, herpetiform dermatitis, seborrheic dermatitis, eczema, psoriasis complicated by secondary bacterial infections.

Active ingredient

Composition

1 g of ointment for external use contains:

Active substances:

mupirocin 20 mg,

betamethasone (in dipropionate form) 500 µg.

How to take, the dosage

Recommended dosage: Apply the ointment in a thin layer on the affected area 2-3 times a day for 5-14 days.

Pick up only on affected areas not exceeding 10 cm in length or 100 cm2 in area.

If patients do not have a clinical response within 3-5 days, they should be re-examined.

Special Instructions

Avoid getting the ointment in the eyes, on mucous membranes, and on open wounds and burns.

As with other antibacterial agents, prolonged use of the ointment may lead to excessive growth of organisms not susceptible to its action, including fungi.

In view of the presence of a potent glucocorticosteroid in the composition of the drug, the drug should be used in the acute period of the disease, for a short time, on a small surface of the skin.

The drug Supirocin B should not be used on the skin of the face due to the possibility of telangiectasia, perioral dermatitis.

In the armpit and groin area, the drug should only be used if absolutely necessary.

The drug should not be used under an occlusive dressing as this can cause atrophy of the epidermis, the appearance of striae and the development of superinfection.

Macrogol-based ointments should not be used on extensive surfaces in conditions accompanied by increased absorption of polyethylene glycol and especially in moderate to severe renal insufficiency.

Contraindications

Cautions:

Macrogol (polyethylene glycol) may be absorbed through the wound surface or other skin injuries and excreted by the kidneys. Macrogol-based ointments should not be used on extensive surfaces in conditions involving increased absorption of polyethylene glycol and especially in moderate to severe renal insufficiency.

For patients with diabetes mellitus, glaucoma, tuberculosis, cataract, long-term use of the ointment on extensive areas of the skin is not recommended. Caution should be exercised when there is atrophy of the subcutaneous tissue, especially in the elderly.

Side effects

Overdose

Symptoms: reversible suppression of hypothalamic-pituitary-adrenal system, manifestations of Cushing’s syndrome, hyperglycemia, glucosuria.

Treatment: symptomatic.

Pregnancy use

The drug is contraindicated in pregnancy and lactation.
