Pharmacodynamics
Sulfadiazine is a broad spectrum antimicrobial bactericidal agent belonging to the sulfonamide group. The drug is active against Gram-positive and Gram-negative bacteria Escherichiacoli, Proteusspp., Staphylococcusspp., Klebsiellaspp.
The bactericidal properties of the ointment are due to the activity of silver ions, which are released in the wound as a result of dissociation of the silver salt of sulfadiazine; the release of silver ions is gradual (moderate dissociation), providing a constant antimicrobial effect. Bactericidal activity of silver ions is complemented by bacteriostatic effect of sulfadiazine (also released during dissociation of silver salt of sulfadiazine).
The drug has moderate osmotic activity and no necrolytic properties.
Pharmacokinetics
About 10% sulfadiazine and 1% silver are absorbed into the peripheral and systemic bloodstream when applied to the wound surface. Application to an extensive wound surface is accompanied by increase of sulfadiazine concentration in blood up to 10-20 µg/ml.
Indications
Treatment of infected surface wounds and burn surfaces with poor exudation, bedsores, abrasions, skin ulcers, skin grafts; as well as prevention of their infection.

How to take, the dosage
Externally. After surgical treatment of the wound and removal of necrotic tissues a thin layer (2-4 mm) of ointment is applied to the damaged surface 1-2 times a day.
The maximum single dose is 300 g. The period of treatment is individual and depends on the nature of the disease and the effectiveness of the treatment used, in general cases lasts up to 3 weeks.
If you forget to apply the drug, use as soon as possible and continue as usual. Do not use a double dose if the previous time the drug was missed.
If symptoms persist or worsen while taking this medicine, tell your doctor.

Special Instructions
When treating patients with hepatic and/or renal insufficiency, regular monitoring of plasma sulfonamide concentrations should be performed.
The parts of the body treated with the drug are recommended to be protected from direct sunlight.
When the ointment is used over a large area, the functional status of the kidneys, liver and blood cells should be monitored; patients should be given plenty of alkaline fluids.
The drug does not affect the ability to drive vehicles and engage in other potentially hazardous activities requiring increased concentration and quick psychomotor reactions.

Contraindications
Pregnancy, lactation, children under 1 year of age; marked deficiency of glucose-6-phosphate dehydrogenase and hypersensitivity to sulfonamide drugs.
The drug should not be used to treat deep purulent wounds and burn surfaces with abundant exudation.
WARNING
Renal/hepatic insufficiency, glucose-6-phosphate dehydrogenase deficiency (risk of hemolysis).

Side effects
Infrequently: leukopenia, which is characterized mainly by a decrease in the number of neutrophils. Rare: skin allergic reactions, disorders of skin pigmentation. Very rare: burning and itching at the application site. When prolonged use on large wound surfaces: systemic side effects typical for sulfonamide drugs. Rarely: allergic reactions. Blood and lymphatic system disorders. Rare: hematopoiesis disorders (agranulocytosis, aplastic and hemolytic anemia, thrombocytopenia, leukopenia). Often: dyspepsia. Rare: hepatitis, hepatocellular necrosis. Rare: toxic nephrosis.
