Pharmacological properties

Pharmacodynamics.

The pharmacological action is due to anthraglycosides (mainly sennosides A and B) as well as other glycosides. When used orally, senna glycosides pass through the stomach and small intestine unchanged and are not absorbed.

In the large intestine, anthraglycosides are broken down by intestinal bacteria into pharmacologically active anthrons and anthranols, which irritate colonic interreceptors, increase peristalsis and accelerate bowel emptying.

Antrons and anthranols prevent absorption of water and electrolytes and promote diffusion of water into the intestinal lumen. Due to the osmotic effect, the stool masses are loosened and their volume increases, contributing to the laxative effect of anthraglycosides.

The laxative effect occurs after 8-10 hours.

Pharmacokinetics.

The metabolites of senna anthraglycosides are practically not absorbed, mostly excreted in the feces as well as in the urine. Also excreted with sweat, penetrate into breast milk.

Indications

– Chronic constipation, hypo and atony of the intestine;

– regulation of stool in hemorrhoids, proctitis, anal fissures;

– colon cleansing before diagnostic and therapeutic procedures.

Composition

Composition per tablet:

The active ingredient:

senna leaf extract dry* (15:1, the extract is 70% ethyl alcohol; with the content of the sum of hydroxyanthracene glycosides converted into senoside B 14.5%) – 93.33 mg;

* Equivalent to 13.5 mg of the sum of hydroxyanthracene glycosides converted into senoside B.

Auxiliary components:

Microcrystalline cellulose, 58.64 mg,

Pregelatinized starch – 58.64 mg,

croscarmellose sodium – 11.50 mg,

How to take, the dosage

Take orally once a day in the evening before going to bed with water.

Adults and children over 12 years: 1 tablet at a time. If there is no effect, the dose can be increased to 2-3 tablets.

The lowest dose necessary to produce soft stools should be used. When adjusting the dose, the same dose should be used for several days; if there is no effect, the dose should be gradually increased by ½ tablet. If defecation does not occur when using the maximum dose within three days, a doctor should be consulted.

The maximum daily dose of the drug is 3 tablets. The drug should not be used for more than 2 weeks. If it is necessary to use the drug as a course of treatment according to the doctor’s recommendation, it is necessary to take a break for two weeks after two weeks of use.

Interaction

In long-term use or use in high doses the drug increases the effect of cardiac glycosides and antiarrhythmic drugs due to reduction of potassium in the body, impairs absorption of tetracyclines.

In concomitant use with thiazide diuretics, glucocorticoids, licorice root preparations, the risk of hypokalemia increases.

Decreases the effectiveness of drugs that are slowly absorbed in the gastrointestinal tract.

Special Instructions

The effective dose of the drug is determined individually. Treatment is started with the lowest possible dose (1 tablet). Development of diarrhea is a sign of drug overdose, in which case the dose should be reduced.

If there is frequent or daily need for laxatives, consult a doctor to determine the cause of constipation. If constipation develops, eating foods rich in plant fiber, drinking plenty of fluids, and exercising are recommended.

Long-term use of the drug may lead to addictive behavior with the necessity of constant increasing of its doses and disturbance of the water-electrolyte balance (in particular, hypokalemia), as well as to intestinal atony. Due to this fact, it is recommended to alternate taking the drug with other laxatives.

The pigmentation of the large intestine may be a consequence of chronic administration of laxatives containing hydroxyanthracene glycosides. This condition is reversible and goes away after you stop taking these drugs.

The urine may turn yellow-brown or reddish after taking the drug. This is not a pathological sign and resolves on its own after withdrawal of the drug.

Impact on ability to drive, operate machinery

The use of the drug does not affect the ability to perform potentially hazardous activities requiring increased concentration and quick psychomotor reactions (driving vehicles and operating moving machinery).

Contraindications

Individual hypersensitivity to senna or other components of the drug;

uterine bleeding, bowel obstruction, spastic constipation, acute inflammatory bowel disease (Crohn’s disease, ulcerative colitis, appendicitis), peritonitis, peptic ulcer of the stomach and duodenum, gastrointestinal bleeding, infected hernia, cystitis, abdominal pain of unknown origin;

severe electrolyte and water balance disorders;

With caution

With caution is used in liver and/or kidney disease, in conditions after cavity surgery, in children from 12 years of age.

Side effects

Gastrointestinal tract: rare – spastic abdominal pain, nausea, vomiting and diarrhea, flatulence, colon atony (with long-term use), pigmentation of the large intestine (deposition of melanin pigment in the colonic mucosa).

Allergic reactions: rare – skin itching, erythema, skin rash.

Urogenital system disorders: yellowish-brown or reddish coloring of urine (due to metabolites excreted by the kidneys; this is not pathological and resolves after discontinuation of the drug), proteinuria, hematuria (with long-term use, especially with high doses).

Metabolic disorders: hypovolemia, hypokalemia (with long-term use, especially in high doses, with diuretics, corticosteroids and cardiac glycosides), secondary hyperaldosteronism.

Others: fatigue, confusion, muscle cramps (in long-term use).

Overdose

Symptoms: abdominal pain, diarrhea, vomiting followed by loss of water and electrolytes.

Treatment: symptomatic, replenishment of fluids and electrolytes (especially potassium) to restore electrolyte balance, which is especially important for children and the elderly.
