Remestip is a vasopressor, hemostatic.
Like the endogenous hormone vasopressin increases the tone of smooth muscles of the vascular wall, causes narrowing of arterioles, veins and venules of visceral organs, which leads to lowering of portal pressure. Terlipressin causes increase of tone and intestinal tube peristalsis and stimulates myometrium contractions.
Pharmacodynamics
Reduces blood flow in the smooth muscle organs, liver and lowers the pressure in the portal system. Causes contraction of the smooth muscles of the esophagus, increases the tone and peristalsis of the intestine, increases the tone of the smooth muscles of the uterus, stimulating the activity of the myometrium both pregnant and non-pregnant uterus. It has no antidiuretic effect.
Indications
- bleeding from the gastrointestinal tract (including. Bleeding from varicose esophageal veins, stomach or duodenal ulcers;
- bleeding from urogenital tract;
- metrorrhage of different genesis; bleeding during childbirth, abortion;
- bleeding during abdominal and pelvic surgery;
- local use in gynecological surgery.

Composition

The active ingredient: terlipressin 100 mcg;
Excipients: sodium chloride; acetic acid; sodium acetate trihydrate; water for injection.
How to take, the dosage
In case of bleeding from the gastrointestinal tract the drug is administered in 1 mg every 4-6 hours.
In case of bleeding from dilated esophageal veins Remestip is administered for 3-5 days. After cessation of bleeding it is recommended to administer the drug for another 2-3 days to prevent recurrence.
In case of bleeding from the urogenital tract, 0.2-1 mg every 4-6 hours. As a rule, a single dose is administered by IV fluids for 1 minute, but infusion of the drug is also possible.
The drug should be administered by IV and only exceptionally by IM.
In gynecological surgery, Remestip may be administered paracervically or intracervically. 400 mcg of the drug in 10 ml of physiological solution is administered. Terlipressin administered in this way begins to take effect after 5-10 minutes. If necessary, the dose may be increased or administered repeatedly.
Interaction
Administration of Remestip together with oxytocin or methylergometrine increases vasoconstrictor and uterotonic action.
Terlipressin enhances hypotensive effect of non-selective β-adrenoblockers.
Combination of Remestip with drugs that slow the heart rate may cause marked bradycardia.
Special Instructions
Caution should be exercised when prescribing the drug in patients with cardiac defects, arterial hypertension, arrhythmias, bronchial asthma, and the elderly.
BP, HR and urine output should be controlled when using the drug (especially in doses over 800 mcg).
Contraindications
- High sensitivity to the drug
- First trimester of pregnancy
- Pregnancy toxicosis
- Epilepsy
Side effects
Most common: pallor of the skin, increased blood pressure, abdominal pain, increased intestinal peristalsis up to abdominal colic nausea, diarrhea, headache, bradycardia.
Rarely: myocardial infarction, heart failure, dyspnea, local necrosis at the injection site.
Overdose
A dose greater than 2 mg (2000 mcg) administered within 4 hours increases the risk of marked hemodynamic disturbances. The recommended dose should not be exceeded. Clonidine (clopheline) should be used to control arterial hypertension. In case of bradycardia, atropine should be administered.
