Topical glucocorticosteroid + keratolytic agent

A combination drug for topical use, which action is due to its constituent components; the drug has anti-inflammatory, anti-allergic, antipruritic, keratolytic action. Betamethasone inhibits release of inflammatory mediators, prevents edge accumulation of neutrophils, reduces inflammatory exudate and cytokine production, reduces migration of macrophages which results in reduction of infiltration and granulation processes.
Owing to the presence of salicylic acid it softens the skin, eliminates stratum corneum and promotes deeper penetration of betamethasone.

Indications
Treatment of subacute and chronic dermatoses accompanied by hyperkeratosis and peeling:
– psoriasis;
– dyshydrotic dermatitis;
– eczema;
– neurodermatitis;
– red squamous lichen;
– ichthyosis and ichthyotic skin lesions;
– atopic dermatitis;
– diffuse neurodermitis;
– simple and allergic dermatitis;
– urticaria;
– erythema multiforme exudative;
– simple chronic lichen (limited neurodermitis);
– dermatoses that cannot be treated with other GCS (especially red warts);
– skin dyshidrosis.
Active ingredient
Composition
100 g:
– betamethasone dipropionate in terms of betamethasone 0.05 g
– salicylic acid 3.0 g
excipients:
liquid paraffin (petroleum jelly),
Vaseline (vaseline white, paraffin white soft).
How to take, the dosage
Externally. It is applied to the affected areas at the rate of: 0.5 cm long column of Rederm ointment on an area of the skin measuring approximately 7 cm à 7 cm, rubbing lightly, in a thin layer 2 times a day.
A different frequency of application can be determined by the doctor, based on the severity of the disease. In mild cases, the ointment is usually enough to use once a day; more frequent use may be necessary for more severe lesions.
If necessary, occlusive dressings are applied and changed every 24 hours.
The duration of treatment depends on the effectiveness and tolerability of therapy and is 2-4 weeks.
If there is no clinical improvement, the diagnosis needs to be clarified.
Interaction
Special Instructions
Long-term use on the face is not recommended; rosacea-type dermatitis, perioral dermatitis, skin atrophy and acne are possible; avoid use in the anogenital area.
The ointment is most suitable for treating patients with dry and fragile skin.
If signs of skin hypersensitivity or irritation associated with use are noted, treatment should be discontinued and the patient should be appropriately treated.
When using the drug over a long period of time, it should be discontinued gradually.
Contraindications
Children under 2 years of age, lactation, hypersensitivity, open wounds, bacterial, viral and fungal skin diseases (pyoderma, syphilis, skin tuberculosis, varicella, herpes, actinomycosis, blastomycosis, sporotrichosis); postvaccination skin reactions, perioral dermatitis, rosacea, trophic ulcers with chronic venous insufficiency, skin tumors (skin cancer, nevus, atheroma, epithelioma, melanoma, hemangioma, xanthoma, sarcoma).
With caution
Hepatic impairment, childhood (older than 2 years), long-term therapy, especially with occlusive dressings.
Side effects
Overdose
Acute overdose is unlikely; however, with excessive or prolonged use of the drug, chronic overdose is possible, accompanied by signs of hypercorticism: hyperglycemia, glucosuria, reversible suppression of adrenal cortical function, manifestation of Cushing’s syndrome.
Treatment.
The appropriate symptomatic treatment is indicated. In case of chronic toxic effects, gradual withdrawal of the drug is recommended.

In children from 2 years of age the drug is prescribed only with strict indications and under medical supervision, because the development of systemic side effects associated with betamethasone is possible. Duration of treatment should be as short as possible. When using the drug on large surfaces and/or under an occlusive dressing, suppression of hypothalamic-pituitary-adrenal system function and development of hypercorticism symptoms may occur, reduction of growth hormone excretion and increased intracranial pressure may be observed.
