How to take, the dosage
Interaction
Concomitant use with ketoconazole, erythromycin may theoretically increase the blood plasma concentration of quetiapine and develop side effects.
Concomitant use with phenytoin, carbamazepine, barbiturates, rifampicin increases clearance of quetiapine and decreases its plasma concentration.
Concomitant use with thioridazine may increase clearance of quetiapine.
Special Instructions
With caution, use in patients with cardiovascular disease and other conditions associated with the risk of arterial hypotension, especially at the beginning of treatment and in the elderly; with a history of seizures.
Quetiapine undergoes active metabolism in the liver. In patients with impaired hepatic and renal function, clearance of quetiapine is reduced by approximately 25%. Therefore, quetiapine should be used with caution in patients with hepatic and/or renal dysfunction.
With caution, use concomitantly with drugs that prolong the QT interval (especially in the elderly); with drugs that have a CNS depressant effect, as well as with ethanol; with potential inhibitors of CYP3A4 isoenzyme (including ketoconazole, erythromycin).
In case of development of MNS during treatment, quetiapine should be discontinued and appropriate treatment should be prescribed.
Long-term use may lead to the development of tardive dyskinesia. In these cases, the dose of quetiapine should be reduced or discontinued.
Cautiously use in combination with other drugs affecting CNS activity, as well as with ethanol.
In male rats (250 mg/kg/day) and mice (250 and 750 mg/kg/day) there was an increased incidence of benign adenomas from thyroid follicular cells, which was related to a known, rodent-specific mechanism of increased hepatic clearance of thyroxine.
Influence on driving and operating machinery
Quetiapine may cause drowsiness, so patients are not advised to perform work that requires concentration and a high rate of psychomotor reactions (including driving vehicles).
Contraindications
Side effects
CNS disorders: headache, drowsiness, dizziness, anxiety; rarely – ANS.
Cardiovascular system: orthostatic hypotension, tachycardia, arterial hypertension.
Digestive system disorders: constipation, dry mouth, dyspepsia, diarrhea, transient increase of liver enzymes activity (ALT, AST, GGT), abdominal pain.
Hematopoietic organs: asymptomatic leukopenia and/or neutropenia; rarely – eosinophilia.
Muscular system disorders: myalgia.
Respiratory system: rhinitis.
Dermatological reactions: skin rash, dry skin.
Hearing organ: ear pain.
Treatment of the urinary system: urinary tract infections.
Metabolism disorders: slight increase in cholesterol and triglycerides in the blood.
Endocrine system disorders: slight dose-dependent reversible decrease of thyroid hormone levels (in particular, total and free T4).
Others: asthenia, low back pain, weight gain, fever, chest pain.
