Indirect-acting anticoagulant. Causes hypoprothrombinemia associated with disruption of prothrombin formation in the liver, also causes a decrease in the formation of VII, IX, X clotting factors, decreased plasma tolerance to heparin, blood lipid content and increased vascular permeability. The action occurs after 8-10 hours and reaches a maximum of 24-30 hours after administration of phenyndione.

Interaction
Concomitant use with ACTH increases the anticoagulant effect.
In concomitant use with dipyridamole there have been cases of increased bleeding, apparently due to decreased platelet adhesion and aggregation by dipyridamole.
The anticoagulant effect is increased when used concomitantly with clofibrate. It is believed that fibrates may increase the affinity of oral anticoagulants to the corresponding receptors or possibly disrupt the metabolism of anticoagulants.
The anticoagulant effect of phenyndione is enhanced when used concomitantly with liothyronine. It is believed that thyroid hormones may increase the affinity of oral anticoagulants for the corresponding receptors.
Concomitant use with miconazole increases the anticoagulant effect of fenindion. The risk of bleeding increases.
Contaneous use with cimetidine increases anticoagulant activity of phenididine due to slowing of its metabolism in the liver under the influence of cimetidine, which is an inhibitor of microsomal liver enzymes.
Concomitant use with ethylestrenol increases anticoagulant activity and causes risk of bleeding.
Special Instructions
The drug should be used with caution in elderly patients, in patients with hepatic and/or renal insufficiency, pulmonary embolism (including in oncology), erosive-ulcerative lesions of the gastrointestinal tract, in pericarditis, in the postpartum period.
The treatment should be carried out under close medical supervision with obligatory systematic investigation of the blood prothrombin content and other clotting factors. Urinalysis should be systematically performed for early detection of hematuria.
In some patients, orange coloring of palms and pink coloring of urine is observed, which is associated with phenindione metabolism.

Digestive system disorders: nausea, diarrhea, liver dysfunction, toxic hepatitis.
Hematopoietic system disorders: inhibition of medullary hematopoiesis.
Urinary system disorders: disorders of renal function.
Cardiovascular system: myocarditis.
Hematous system disorders: with prolonged use – micro- and macrohematuria, bleeding from the mouth and nasopharynx, gastrointestinal tract, muscle haemorrhage.
CNS disorders: headache.
Allergic reactions: skin rash, dermatitis, eosinophilia, fever.
