Pancreatin Forte is a digestive enzyme, compensates for the lack of secretory function of the pancreas and biliary function of the liver, improves digestion, has proteolytic, amylolytic and lipolytic action.

Included pancreatic enzymes (lipase, alpha-amylase, trypsin, chymotrypsin) help in the breakdown of proteins into amino acids, fats into glycerol and fatty acids, starch into dextrins and monosaccharides.

Trypsin suppresses stimulated pancreatic secretion with an analgesic effect.

The enzyme hemicellulase promotes the breakdown of plant fiber, which also improves digestion and reduces the formation of gas in the intestines. Bile extract has a choleretic effect, helps emulsify fats, improves absorption of fats and fat-soluble vitamins and increases lipase activity.

Pharmacokinetics

Pancreatic enzymes are released from the dosage form in the alkaline environment of the small intestine because they are protected from the action of gastric juice by an enteric coating.

The maximum enzymatic activity of the drug is observed 30-45 minutes after oral administration.

Indications

Pancreatic replacement therapy in external pancreatic insufficiency: chronic pancreatitis, pancreatectomy, condition after radiation, dyspepsia, cystic fibrosis; flatulence, noninfectious genesis diarrhea.

Digestive disorders (condition after resection of the stomach and small intestine); to improve digestion in persons with normal gastrointestinal tract function in case of poor diet (consumption of fatty foods, large amounts of food, irregular meals) and with disorders of chewing function, sedentary lifestyle, prolonged immobilization. Remheld’s syndrome (gastrocardia syndrome).

Preparing for radiological examination and ultrasound of the abdominal cavity.

How to take, the dosage

Ingestion, swallowed whole (without chewing), during or immediately after a meal, with plenty of liquid (if possible alkaline: water, fruit juices). Dose of the drug is set individually (in terms of lipase) depends on age and degree of pancreatic insufficiency.

It is not recommended to exceed the maximum daily dose of 15,000-20,000 IU of lipase/kg, especially in patients with cystic fibrosis.

The average dose for adults is 150,000 IU/day; with complete failure of external pancreatic secretory function it is 400,000 IU/day, which corresponds to the daily lipase requirement of an adult.

The duration of treatment may vary from a single dose or a few days (when digestion is disturbed due to errors in the diet) to several months or even years (if permanent replacement therapy is necessary). Adults – 3-4 tablets 3 times a day. Higher doses are prescribed by the doctor. 2 tablets 2-3 times a day for 2-3 days before X-ray or ultrasound examination.

In children over 3 years of age, 100,000 IU/day (for lipase) divided into 3-4 doses.

Contraindications

Hypersensitivity (including intolerance to pork), acute pancreatitis, exacerbation of chronic pancreatitis.

Side effects

Allergic reactions, rarely – diarrhea or constipation, nausea, discomfort in the epigastric region, abdominal pain, decreased endogenous bile acid synthesis. When prolonged use in high doses – hyperuricosuria; when using high doses in patients with cystic fibrosis – strictures in the ileocecal region and in the ascending colon, manifested by symptoms of intestinal obstruction.
