Timolol is a non-selective beta-adrenoreceptor blocker. It has no membrane stabilizing or sympathomimetic activity. It affects both elevated intraocular pressure and normal pressure by reducing the amount of aqueous humor through its production. It begins to act one-third of an hour after instillation and continues for 24 hours. The maximum effect is observed after 1-2 hours.
Injection the drug is quickly absorbed into the eyeball. The systemic effect is minimal and occurs by absorption of Timolol through the conjunctiva, nasal mucosa, tear ducts.
Pharmacokinetics
In local administration it quickly penetrates through cornea, in small amount it enters systemic bloodstream due to absorption through conjunctival vessels, nasal mucosa and lacrimal tract.

Composition
The active component of the drug is timolol (timolol maleate), which in 1 ml of solution contains 5 mg
Additional ingredients include:
conservative benzalkonium chloride (0.015),
betacyclodextrin,
water for injection,
p> EDTA disodium,
dihydrate disodium hydrophosphate,
sodium chloride,
dihydrate sodium dihydrophosphate.
The solution itself is clear colorless to slightly yellowish and has no mechanical inclusions.
How to take, the dosage
Conjunctival.
In the early stages of treatment, Ocupres-E is instilled into the diseased eye twice daily with 1 drop. When intraocular pressure normalizes, the frequency of administration should be reduced to once a day.
If further decrease in intraocular pressure is deemed necessary, then concomitant use of myotics or systemic administration of carbonic anhydrase inhibitors may be prescribed.
The treatment with the drug is usually prolonged.
When the intraocular pressure reaches a satisfactory level, the patient may be prescribed Ocupres-E once daily.
Breaks or dosage changes should only be made with the prescription of the treating physician.
Interaction
The co-administration of Ocupres-E with eye drops containing epinephrine may cause dilation of the pupil.
The specific effect of the drug on reducing intraocular pressure is enhanced by the simultaneous use of eye drops containing epinephrine and pilocarpine; two beta-adrenoblockers should not be injected into the eyes.
Lowering of blood pressure and slowing of heart rate may be potentiated when the drug is used concomitantly with slow calcium channel blockers, reserpine and other beta-adrenoblockers.
Simultaneous use with insulin or oral antidiabetic agents may lead to hypoglycemia.
Timolol increases the effect of peripheral muscle relaxants, therefore, the drug must be withdrawn 48 hours before the planned surgical intervention with the use of general anesthesia.

Special Instructions
With caution is used in patients with liver and kidney function abnormalities, with diabetes mellitus (especially of labile course). With long-term use increases the level of triglycerides in the blood plasma.
The safety and effectiveness in children has not been studied.
In ophthalmology it is used for a long time, therefore during the period of treatment at least once in 6 months a corneal examination should be carried out, the lacrimation function and the state of the fields of vision should be controlled.
Impact on driving and operating machinery
At the time of treatment, one must refrain from potentially hazardous activities requiring increased attention and quick psychomotor reactions.
When used in ophthalmology immediately after instillation vision acuity may decrease and psychomotor reactions may slow down, so refrain for 30 minutes from engaging in potentially hazardous activities requiring increased attention.
Contraindications
Ocupres-E is not prescribed if you are hypersensitive to the components of the drug. It should also not be used if there is a history of:
Caution should be exercised in cerebrovascular, heart failure, patients with hypoglycemia, diabetes, thyrotoxicosis, myasthenia gravis, together with systemic beta-blockers.
The components of the drug Ocupres-E are able to pass through the placental blood flow and into the breast milk. Because of this, the possible risks and potential benefits should be evaluated before prescribing the drug.
Side effects
Both local and systemic effects may occur with Ocupres-E drops.
The former include: mucosal redness, corneal edema, burning, irritation, itching, photophobia, pinpoint keratopathy, lacrimation, diplopia, corneal hypoesthesia, dryness, ptosis. In case of operations against glaucoma (fistulizing), retinal detachment may form in the postoperative period.
Systemic reactions are similar to the side effects of all beta-blockers (bronchospasm, heart failure, headache, etc.). Allergic reactions (eczema, urticaria) also sometimes occur.
The most dangerous is apnoea, which occurs when the drug is prescribed to newborns.
Overdose
In case of overdose of the drug Ocupres-E, there are common reactions to overdose of beta-blockers.
In this case, the eyes should be flushed with saline or water and a doctor should be consulted.
The treatment is symptomatic.
