Micardis is a hypotensive.

Blocks angiotensin II receptors (type AT₁) and eliminates its vasoconstrictor effect. Reduces systemic BP, plasma concentrations of aldosterone.

Pharmacokinetics

In oral administration, it is rapidly absorbed from the gastrointestinal tract. Bioavailability is 50%. In concomitant use with food AUC decreases from 6% (at a dose of 40 mg) to 19% (at a dose of 160 mg). In 3 hours after the dose, plasma concentrations level off regardless of whether the drug is taken on an empty stomach or with food. C_max and AUC are 3 and 2 times higher in women compared to men, respectively, with no significant effect on concentration. Binding to plasma proteins is 99.5%, mainly to albumin and alpha₁ glycoprotein. Mean apparent volume of distribution in the equilibrium stage is 500 L. Metabolized by conjugation with glucuronic acid. Metabolites are pharmacologically inactive. T_1/2 is greater than 20 h. Total plasma Cl is high (900 ml/min) compared to hepatic blood flow (about 1500 ml/min). Excreted through the intestine unchanged, excretion by the kidneys is less than 2%.

The maximum effect develops 3 hours after a single dose. Hypotensive effect lasts more than 24 hours, including the last 4 hours before taking the next dose. Stable clinical result is reached after 4-8 weeks of course use and is long maintained. On abrupt discontinuation of therapy BP gradually (over several days) returns to baseline values without the manifestation of “ricochet” hypertension.

How to take, the dosage

Ingestion, regardless of meals. In adults, 40 mg once daily. In some patients, therapeutic effect may be achieved using a dose of 20 mg/day. If BP does not decrease to the desired level, the dose may be increased to 80 mg once a day. Maximum efficacy of hypotensive action of the drug is usually observed 4 to 8 weeks after the start of treatment.

In patients with severe arterial hypertension, up to 160 mg/day or in combination with hydrochlorothiazide 12.5-25 mg/day.

Interaction

Thiazide diuretics (e.g. hydrochlorothiazide) enhance the hypotensive effect of telmisartan. Telmisartan increases the hypotensive effect of other antihypertensive drugs, increases the blood concentration of digoxin.

Special Instructions

Impact on the ability to drive a car and machinery
Special studies of the effect of the drug on the ability to drive a car and machinery have not been conducted. However, when driving motor vehicle and machinery, one should remember about the possibility of dizziness and somnolence when using the drugs used for treatment of arterial hypertension.

Contraindications

Hypersensitivity, biliary tract patency disorders, marked liver or renal dysfunction, hereditary fructose intolerance, pregnancy, breastfeeding, childhood and adolescence.

Side effects

Nervous system disorders: headache, dizziness, fatigue, insomnia, anxiety, depression, seizures.

Respiratory system disorders: upper respiratory tract infections (including pharyngitis, sinusitis, bronchitis), cough.

Cardiovascular system: marked BP lowering, bradycardia, tachycardia, chest pain.

Digestive system disorders: nausea, dyspepsia, diarrhea, abdominal pain, increased liver transaminase activity.

Motor system disorders: myalgia, arthralgia, lower back pain, tendinitis-like symptoms.

Uses of the urinary system: peripheral edema, urinary infections, hypercreatininemia.

Allergic reactions: skin rash, etc.

Laboratory measures: rarely – hyperkalemia, anemia or hyperuricemia.

Others: flu-like syndrome, rarely erythema, pruritus, syncope, dyspnea, eosinophilia, thrombocytopenia, angioedema, urticaria.

Overdose

Symptoms: marked BP decrease.

Treatment: symptomatic therapy; hemodialysis is ineffective.
