Pharmacological properties
Pharmacodynamics.
The enzyme drug that improves digestion. Pancreatin is a powder from porcine pancreatic glands, which contains other enzymes along with the external pancreatic enzymes – lipase, amylase, protease, trypsin and chymotrypsin.
The pancreatic enzymes in the drug facilitate the breakdown of proteins, fats and carbohydrates, which leads to their more complete absorption in the small intestine. Trypsin suppresses stimulated pancreatic secretion with analgesic effect. Maximum enzymatic activity of the drug is noted 30-45 min after oral administration.
Pharmacokinetics.
The tablets of Mesim® Pro are covered with an acid-proof coating which is insoluble under the action of gastric hydrochloric acid and by this means protects the enzymes contained in the drug from inactivation. Solution of the coating and release of enzymes occurs at pH values close to neutral or slightly alkaline.
Indications
- replacement therapy in external pancreatic function insufficiency (including. Chronic pancreatitis, cystic fibrosis)
- chronic inflammatory-dystrophic diseases of the stomach, intestines, liver, gall bladder;
- .conditions after resection or irradiation of the GIT organs, accompanied with disturbances in digestion, flatulence, diarrhea (as a part of combined therapy);
- GIT disorders of functional character (with intestinal infectious diseases, irritable bowel syndrome, etc.
- to improve digestion in patients with normal gastrointestinal function in case of irregularities in diet;
- preparation for radiological and ultrasound studies of the abdominal cavity.

How to take, the dosage
The dose of the drug Mesim ® Pro is set individually depending on the severity of the disease and the food composition.
If not otherwise stated, the average single dose for adults is 2-4 tablets of Mesim ® Pro per meal. It is recommended to take half or third of the single dose at the beginning of a meal and the rest during the meal. The drug is taken orally without chewing and with plenty of fluids.
The dose can be increased and should only be taken under medical supervision, taking into account any reduced symptoms (e.g., steatorrhea, abdominal pain). The maximum daily dose is 15000-20000 units of lipase/kg body weight.

Children The dosing regimen is determined by the doctor depending on the severity of the disease and the food composition at a rate of 500-1000 IU of lipase/kg of body weight for each meal of the child.
The duration of treatment may vary from a few days (for digestive disorders and errors in the diet) to several months or even years (if permanent replacement therapy is necessary).
Interaction
When taking drugs containing pancreatin, folic acid absorption may be reduced. The effect of sugar-lowering drugs (acarbose, miglitol) may be reduced when concomitant use with pancreatin.
Concomitant use of pancreatin with iron preparations may decrease absorption of the latter. Concomitant use of antacids containing calcium carbonate and/or magnesium hydroxide may reduce the effectiveness of the drug.
Special Instructions
In acute pancreatitis or exacerbation of chronic pancreatitis (at the stage of exacerbation subsiding), during restorative dietary feeding, it is reasonable to prescribe Mezim® Pro against the background of existing or remaining insufficiency of pancreatic gland function.
The drug Mesim® Pro, taking into account the solid indivisible dosage form, is contraindicated in children under 3 years of age.
Influence of the drug on driving and operating ability
Preparation Mesim ® Pro has no effect on the speed of psychomotor reactions and on the ability to perceive or assess the situation.
Synopsis
Pink coated flat-cylindrical tablets with almost flat-parallel surfaces and beveled edges, the breakage may have brown blotches.
Contraindications
- acute pancreatitis;
- acute chronic pancreatitis;
- high sensitivity to pancreatin or other ingredients of the drug;
- hereditary galactose intolerance, lactose deficiency or glucose-galactose malabsorption syndromehereditary galactose intolerance, lactase deficiency or glucose-galactose malabsorption syndrome;
- children under 3 years of age (indivisible dosage form).

Side effects
No side effects or complications have been found, even with long-term and regular use of Mesim® Pro in patients with impaired pancreatic function.
In single cases after pancreatin administration allergic reactions may develop, rarely – diarrhea or constipation, nausea, discomfort in epigastric region. Patients with cystic fibrosis may occasionally develop hyperuricosuria (elevated plasma levels of uric acid) with prolonged use in high doses, and stricture formation in the ileocecal region and ascending colon may occur.
Overdose
There are no data on cases of overdose and intoxication with the drug. Hyperuricosuria, hyperuricemia, constipation in children are possible. Treatment: drug withdrawal, symptomatic therapy.
