Mendilex (biperiden) is a choline-blocking drug of central action, with moderately pronounced peripheral action.
Indications
Parkinson’s syndrome (as part of combined therapy), including the initial stages of Parkinson’s disease (monotherapy).
Extrapyramidal disorders (tremor, muscle rigidity, torsional dystonia and local forms of dystonia, including paroxysmal dyskinesia, chorea hyperkinesis, various forms of tics, myoclonia and hyperkinesis, dystonia of the muscles of the extremities, trunk, neck, face, akinesia, akathisia) caused by medications (in particular neuroleptics).
Active ingredient
Interaction
Enhances the effect of M-cholinoblockers, antihistamines, anti-Parkinsonian and antiepileptic drugs, weakens the effect of metoclopramide.
Incompatible with ethanol.
Levodopa increases M-cholin-blocking effects, quinidine increases risk of dyskinesias.
Directions for use
Special Instructions
The use of biperiden may cause neuromuscular block, with subsequent muscle weakness and paralysis.
Alcohol should not be consumed during treatment with biperiden.
In susceptible patients, confusion, euphoria, and motor agitation are observed when the anticholinergic drug is administered at the recommended therapeutic doses.
Impact on driving, operating machinery
The drug has a strong effect on psychomotor reactions, therefore, patients taking the drug should refrain from engaging in potentially hazardous activities requiring rapid mental and motor reactions.
Contraindications
Hypersensitivity, myasthenia gravis, closed-angle glaucoma, intestinal obstruction, urinary tract obstruction, paralytic intestinal obstruction or intestinal atony, gastroesophageal reflux, nonspecific ulcerative colitis, toxic megacolon, acute bleeding.
With caution.
Elderly patients and patients with prostatic hypertrophy, tachyarrhythmia, congestive heart failure, coronary heart disease, arterial hypertension, ileostomy/colostomy, epilepsy.
Children under 18 years of age (effectiveness and safety of the drug in children have not been established).
Side effects
Overdose
The symptoms of overdose are similar to those of atropine poisoning: mydriasis, dry skin, hyperemia of the face, dry mouth and upper airways, increased body temperature, tachycardia, cardiac arrhythmias, impaired bowel peristalsis, urinary retention, delirium, disorientation, anxiousness, hallucinations, confusion, ataxia, aggressiveness and seizures.
In severe poisoning, stupor, coma, respiratory and cardiac arrest, and death are possible.
Care: gastric lavage, symptomatic therapy. It is recommended to prescribe physostigmine, as a specific antagonist in a dose of 1 mg in m/m or by slow IV infusion.
This dose may be repeated every 20 minutes, but the total should not exceed 4 mg per day. Because physostigmine is rapidly metabolized, additional infusions of physostigmine are sometimes necessary every 1-2 hours. For children and elderly patients, half the standard dose is recommended.
The patient should be monitored by a physician for at least 8-12 hours after the last appearance of symptoms.
