Mendilex (biperiden) is a choline-blocking drug of central action, with moderately pronounced peripheral action.

Indications

Parkinsonism syndrome (as part of combination therapy), including the initial stages of Parkinson’s disease (monotherapy).

Extrapyramidal disorders (tremor, muscle rigidity, torsion dystonia and local forms of dystonia, including paroxysmal dyskinesia, chorea hyperkinesia, various forms of tics, myoclonus and hyperkinesis, dystonia of the muscles of the limbs, trunk, neck, face, akinesia, akathisia) caused by drugs (in particular antipsychotics).

Pharmacological effect

Mendylex (biperiden) is an anticholinergic drug of central action, with moderate peripheral action.

Special instructions

The use of biperiden can cause neuromuscular blockade, followed by muscle weakness and paralysis.

During treatment with biperiden you should not drink alcohol.

In susceptible patients, confusion, euphoria, and motor agitation are observed when an anticholinergic drug is prescribed in recommended therapeutic doses.

Impact on the ability to drive vehicles and machinery

The drug has a strong effect on psychomotor reactions, so patients taking the drug should refrain from engaging in potentially hazardous activities that require rapid mental and motor reactions.

Pregnancy

Prescription during pregnancy is possible if the benefit to the mother outweighs the possible risk to the fetus.

Contraindications

Hypersensitivity, myasthenia gravis, angle-closure glaucoma, intestinal obstruction, urinary tract obstruction, paralytic ileus or intestinal atony, gastroesophageal reflux, ulcerative colitis, toxic megacolon, acute bleeding.

With caution.

Elderly patients and patients suffering from prostatic hypertrophy, tachyarrhythmia, congestive heart failure, coronary heart disease, arterial hypertension, ileostomy/colostomy, epilepsy.

Children under 18 years of age (the effectiveness and safety of the drug in children has not been established).

Side Effects

Mostly side effects develop as a result of the anticholinergic effect of the drug.

Interaction

Strengthens the effect of M-anticholinergics, antihistamines, antiparkinsonian and antiepileptic drugs, weakens the effect of metoclopramide.

Incompatible with ethanol.

Levodopa increases the M-anticholinergic effect, quinidine increases the risk of developing dyskinesias.

Overdose

Symptoms of overdose are similar to those that occur with atropine poisoning: mydriasis, dry skin, facial flushing, dry mouth and upper respiratory tract, elevated body temperature, tachycardia, cardiac arrhythmias, weakened intestinal motility, urinary retention, delirium, disorientation, anxiety, hallucinations, confusion, ataxia, aggressiveness and convulsions.

In case of severe poisoning, stupor, coma, respiratory and cardiac arrest, and death are possible.

Providing assistance: gastric lavage, symptomatic therapy. It is recommended to prescribe physostigmine as a specific antagonist at a dose of 1 mg IM or by slow IV infusion.

This dose can be repeated every 20 minutes, but the total amount should not exceed 4 mg per day. Since physostigmine is rapidly metabolized, additional infusions of physostigmine every 1-2 hours are sometimes necessary. For children and elderly patients, half the standard dose is recommended.

The patient should be under medical supervision for at least 8-12 hours after the last onset of symptoms.

Inside. For Parkinson’s disease and parkinsonism, the initial dose is 2 mg per day, divided into two doses.

List A. Store the drug in a place protected from light at a temperature of 15-25°C.

Shelf life

5 years. Do not use after expiration date.
